原发性闭角型青光眼发病影响因素调查研究

摘　　要：目的探讨原发性闭角型青光眼患者发病影响因素,并为早期采取针对性的预防措施提供参考依据。方法采用发病影响因素自编问卷收集2010年12月~2012年12月浙江嘉兴武警医院眼科收治的56例原发性闭角型青光眼患者和同期112例健康体检者的临床资料,采用描述性统计和二分类Logistic回归分析原发性闭角型青光眼患者发病影响因素。结果多因素Logistic回归分析显示,高龄（OR=5.644,P=0.002）、高血压病（OR=12.947,P=0.000）、糖尿病（OR=12.869,P=0.000）、A型行为模式（OR=6.193,P=0.000）、血液黏稠度异常（OR=12.855,P=0.000）、睫状体与晶状体间的距离短（OR=7.124,P=0.000）、浅前房（OR=13.850,P=0.000）、角膜小（OR=8.352,P=0.000）、晶状体较厚（OR=4.391,P=0.000）、眼轴较短（OR=8.159,P=0.000）和虹膜与晶体接触距离增大（OR=5.523,P=0.001）是原发性闭角型青光眼患者发病的危险因素,而近距离看电视（OR=0.421,P=0.000）是原发性闭角型青光眼患者发病的保护因素。结论原发性闭角型青光眼的发病受多种因素影响,应关注高危人群,采取针对性措施降低原发性闭角型青光眼的发病率,改善预后。Objective To investigate the influential factors of primary angle-closure glaucoma,in order to provide ba sis for adopting corresponding prevention countermeasures.Methods The clinical data of 56 primary angle-closure glaucoma who were treated in the Ophthalmology Department of Jiaxing Armed Police Hospital from December 2010 to December 2012 and 112 healthy control were collected by the self-made general data questionnaire,then the influen tial factors of primary angle-closure glaucoma were analyzed by descriptive statistics and binary classification Logistic regression analysis.Results The multiple Logistic regression analysis showed that advanced age（OR = 5.644,P = 0.002）,hypertension（OR = 12.947,P = 0.000）,diabetes（OR = 12.869,P = 0.000）,type A behavior pattern（OR = 6.193,P = 0.000）,abnormal blood viscosity（OR = 12.855,P = 0.000）,short distance between the ciliary body and the lens（OR = 7.124,P = 0.000）,shallow anterior chamber（OR = 13.850,P = 0.000）,small cornea（OR = 8.352,P = 0.000）,thick lens（OR = 4.391,P = 0.000）,short axis oculi（OR = 8.159,P = 0.000） and increasing contact distance between iris and crystal（OR = 5.523,P = 0.001） were risk factors of primary angle-closure glaucoma,while,watching TV in the distance（OR = 0.421,P = 0.000） was the protective factor of primary angle-closure glaucoma.Conclusion The incidence of primary angle-closure glaucoma is affected by various factors.In daily work,the high risk population should be paid close attention to,corresponding measures should be taken to reduce the incidence rate of primary angle-closure glaucoma,in order to improve the prognosis of patients.
