摘 要:目的对原发性开角型青光眼(POAG)病人进行视网膜缺血监测(RIM)检查及分析,探讨RIM在POAG中的临床应用价值。方法对10例20眼POAG病人及正常人10例20眼(对照组)进行RIM检查,记录振幅及潜伏期;POAG病人并行光学相干断层扫描(OCT)检查,记录视网膜神经纤维层平均厚度。结果与对照组相比,POAG中、晚期病人RIM振幅降低,潜伏期延长,差异有显著性(F=57.256、17.976,t=5.517~9.959,P<0.05);早期病人RIM振幅、潜伏期差异无显著性(P>0.05)。POAG病人中期与晚期组间比较,振幅、潜伏期差异无统计学意义(P>0.05)。RIM振幅与视网膜神经纤维层平均厚度呈正相关(r=0.735,P<0.05),潜伏期与神经纤维层平均厚度呈负相关(r=-0.629,P<0.05)。结论 RIM尚不能作为POAG病人早期诊断的指标;RIM振幅与潜伏期出现异常时,POAG已进展到中期,视网膜缺血已较为严重,应积极治疗。Objective To evaluate the clinical value of retinal ischemia monitoring (RIM) in primary open angle glauco- ma (POAG). Methods RIM was carried out in 10 patients (20 eyes) with POAG and 10 healthy individuals (20 eyes), the am plitude and latency period were recorded; for POAG, optical coherence tomography (OCT) was done as well, and the average thickness of retinal nerve fibre layer (RNFL) recorded. Results Compared with the controls, the amplitude of RIM was lower and the latency was longer in moderate or advanced POAG(F =57.256,17.976;t = 5.517--9.959 ;P 〈0.05); No significant differ ences were found of that between early POAG and the control (P〉0.05). A comparison among moderate and advanced POAG, showed the differences of amplitude and latency were not significant (P〉0.05). The amplitude of RIM was positively correlated with the average thickness of RNFL(r=0.735,P〈0.05) ; and that in latency was negatively correlated (r= -0.629,P〈0.05). Conclusion RIM is still can't be used as a marker for early diagnosis of POAG. When the amplitude and latency of RIM are abnor- mal, POAG already reached its metaphase with severe retinal ischemia that needs to be treated actively.
