临床表现似脂溢性皮炎的基底细胞癌1例

摘　　要：患者女,44岁。左鼻翼红斑2年,缓慢扩大3个月。皮肤科情况:左鼻翼淡红色斑,大小0.8cm×0.8cm,表面无破溃,触之有轻度浸润感,边界清楚。皮损组织病理示:肿瘤在真皮全层呈团块状浸润,瘤细胞核大,嗜碱性,癌巢与周围组织间有裂隙,周边细胞呈栅栏状排列。免疫组化标记:瘤细胞P63(+),Ber-Ep4(+),CK7(-),EMA(-),CEA(-),CD10(-),S-100(-);Ki67阳性率约20%。诊断:基底细胞癌。行手术治疗,预后良好。A 44-year-old female patient have presented with reddish macule on the leit nose for two years, expanded slowly for three months. Physical examination： reddish macule on the left nose, sized 0. 8cm x 0. 8cm with clear boundary, no surface rupture and a mild infiltration sense. Pathological examination： the tumor showed massive infiltration in the dermis, tumor cells were basophilia with prominent nuclei, the tumor cells located in the edge were ranged palisading with space between the tumor and surrounding tissue. Immunohistochemical staining showed that the tumor cell expressed P63, Ber-Ep4 and positive rate of Ki67 was about 20%, did not express CK7, EMA, CEA, CD10 and S-100. The diagnosis was basal cell carcinoma. The patient was treated with surgical resection and the prognosis is good.
