6种抗精神病药物治疗12周对体重、血糖、血脂及催乳素的影响

摘　　要：目的比较齐拉西酮、利培酮、奥氮平、喹硫平、氯氮平、舒必利6种抗精神病药物治疗12周对体重、血糖、血脂及催乳素的的影响。方法对240例女性分裂症患者随机分组应用6种药物治疗。治疗前与治疗12周测量体重、血糖、总胆固醇及催乳素。结果氯氮平、奥氮平组体重及空腹血糖、总胆固醇治疗后与治疗前相比差异极具显著性;利培酮、喹硫平组治疗后与治疗前相比差异有显著性;齐拉西酮、舒必利组与治疗前比较差异没有显著性。依次可排列为:氯氮平>奥氮平>利培酮>喹硫平>舒必利≥齐拉西酮。催乳素舒必利组与利培酮组治疗后与治疗前比较差异有显著性;齐拉西酮、奥氮平、氯氮平、喹硫平组治疗前后比较差异没有显著性。结论在常用的抗精神病药物中齐拉西酮治疗分裂症患者时对内分泌影响的副反应明显最小。可能增加女性患者的服药依从性。Objective To observe the impact on weight gain, fasting blood glucose , blood lipids and pro- lactin of ziprasidone, risperidone, olanzapine, quetiapine, clozapine and sulpiride for treating schizophrenia pa-tients for 12 weeks. Methods 240 female schizophrenic patients were randomly divided into ziprasidone, risperi- done, olanzapine, quetiapine, clozapine and sulpiride groups. Body weight, fasting blood glucose, blood lipids and prolactin were measured the baseline and after 12 weeks trearment. Results Compared with baseline, the body weight, fasting blood-glucose （FBG） and total cholesterol were increased significantly after 12 weeks treat- ment in clozapine, olanzapine, risperidone and quetiapine （P〈0. 01 ）, no significant change were found in done and sulpiride. Compared with baseline, the prolactin level was increased significantly in risperidone; ziprasi- no sig- nificant changes were found in ziprasidone, olanzapine, quetiapine and clozapine,. Conclusion Ziprasidone is minimal in endocrine side effects on female schizophrenic patients, it may improve the female patient＇s medication compliance.
