摘 要:目的探讨泌尿系统恶性肿瘤患者化疗后感染调查和影响因素,为采取针对性的预防干预措施提供参考依据。方法收集泌尿外科2011年2月-2013年2月收治的290例泌尿系统恶性肿瘤患者临床资料,根据泌尿系统恶性肿瘤患者有无感染,分为感染组与非感染组,运用SPSS19.0统计软件进行描述性统计和二分类logistic回归分析泌尿系统恶性肿瘤化疗后感染影响因素。结果 290例患者中发生感染31例,感染率为10.69%,以呼吸道感染为首15例,占48.39%,多因素logistic回归分析发现,侵入性操作多(β=3.084,OR=21.847)、外周血白细胞数低(β=3.188,OR=24.244)和住院时间>30d(β=2.608,OR=13.574)是泌尿系统恶性肿瘤患者化疗后感染的影响因素,专科护理人员护理(β=-2.849,OR=0.058)是泌尿系统恶性肿瘤患者化疗后感染的保护因素。结论泌尿系统恶性肿瘤患者化疗后感染率较高,其受多方面因素影响。OBJECTIVE To investigate the influencing factors of infections in the urinary tract malignant tumor patients after chemotherapy so as to provide reference for taking targeted interventions. METHODS The clinical data of two hundred and ninety urinary tract malignant tumor patients after chemotherapy who were treated in the urinary surgical department in the hospital from Feb 2011 to Feb 2013 were collected by the self-made general data questionnaire, they were divided into the infection group and the non--infection group according to the status of infections in the urinary tract malignant tumor patients after chemotherapy, then the descriptive statistics were taken by using SPSS16.0, and the binary classification logistic regression analysis was performed to find out the impact factors of the infections in the urinary tract malignant tumor patients after chemotherapy after chemothera- py. RESULTS Of totally 290 cases of patients, the infection occurred in 31 cases with the incidence rate of 10.69%, among which the cases with respiratory tract infections ranked the first place (15 cases, 48.39%). The multivariate logistic regression analysis showed that the frequent invasive operations (β= 3. 084, OR= 21. 847), low peripheral blood white blood cell count(β= 3. 188, OR= 24. 244), and length of hospital stay more than 30 days (β=2. 608,OR=13. 574) were the influencing factors of the infections in the urinary tract malignant tumor patients after chemotherapy and that the nursing of the professional nursing staff (β=- 2. 849,OR = 0. 058) was the protective factor of the infections in the urinary tract malignant tumor patients after chemotherapy. CONCLUSION The infection rate is relatively high in the urinary tract malignant tumor patients after chemothera- py, which is affected by various factors.
