白细胞介素-21受体基因多态性与β-内酰胺类抗生素过敏的关联性分析

摘　　要：目的分析白细胞介素-21受体(IL-21R)基因多态性与β-内酰胺类抗生素过敏的关联性。方法随机选取甘肃地区汉族人群201例β-内酰胺类抗生素过敏病例样本和209例正常对照样本,应用聚合酶链反应-限制性片段长度多态性技术,采用病例-对照方法研究β-内酰胺类抗生素过敏与IL-21R的rs963154和rs2285452位点关联性,并通过测序进行验证。结果 IL-21R基因的rs963154位点CC、CG、GG基因型在病例组中的分布频率分别为0.9%、23.4%和75.7%,在对照组中的分布频率分别为0.5%、19.3%和80.2%;rs2285452位点AA、AG、GG基因型在病例组中的分布频率分别为0.9%、20.7%和78.4%,在对照组中的分布频率分别为1.1%、17.0%和81.9%。rs963154位点C、G等位基因及rs2285452位点A、G等位基因在病例组和对照组之间的频率分布差异分别为[P=0.360,OR=0.743(0.467～1.184)];[P=0.524,OR=0.173(0.471～1.146)]。进行性别分组后其基因型频率和等位基因频率分布差异仍无统计学意义。结论 IL-21R基因rs963154、rs2285452基因多态性与甘肃地区汉族人群β-内酰胺类抗生素过敏无显著相关性。Objective To identify the correlation between polymorphisms of interleukin 21 receptor （IL-21R） and β-lactam allergy. Methods 201 patients with drug allergy against β-lactam as allergy group and 209 patients without allergy as control group were selected with a case-control method in the Han nationality of Gansu to investigate the correlation between the IL-21R rs943154 and rs2285452 and β-lactam allergy by using the polymerase chain reaction-restriction fragment length polymorphism method. Sequencing was used to verify the results. The statistical data was treated by the SPSS 17.0 software. Results The frequency distribution of IL-21R rs963154 genotypes in the β-lactam allergy group and control group were as follows： CC： 0.9% and 0.5%. CG： 23.4% and 19.3%, GG： 75.7% and 80.2%. The frequency distribution of IL-21R rs2285452 genotypes in the β-lactam allergy group and control group were as follows： CC： 0.9% and 0.5%, CG： 23.4% and 19.3%, GG： 75.7% and 80.2%, respectively. No significant differences were found in the allele frequencies of rs963154 C, G and rs2285452 A, G between the two groups [P=0.360, OR=0.743 （0.467±1.184）] and [P=0.524, OR=0.173 （0.471±1.146）]. Nor gender stratification was found. Conclusion There may be no obvious correlation between genetic polymorphisms of IL- 21 receptor and the risk of β-1actam allergy of Han nationality in Gansu.
