胸腔镜肺叶切除术对非小细胞肺癌患者术后免疫功能的影响

摘　　要：目的:探讨胸腔镜肺叶切除术对非小细胞肺癌患者术后免疫功能的影响。方法:采用前瞻性对照研究,将2010年1月-2012年12月本院胸心外科收治的60例非小细胞肺癌患者随机分成两组,胸腔镜组(V组,30例)和传统开胸组(T组,30例)。分析两组手术时间、术中出血量及细胞免疫功能等指标。结果(:1)V组肿瘤大小、术中出血量、输血量、手术时间、住院天数均短于或小于T组(P<0.05);(2)术前两组CD4+、CD8+、CD4+/CD8+差异均无统计学意义;(3)术后1周T组CD4+、CD8+、CD4+/CD8+较术前显著下降(P<0.05),而V组仅CD4+较术前显著下降(P<0.05);(4)术后1周T组CD4+、CD8+、CD4+/CD8+均较V组显著下降(P<0.05)。结论:胸腔镜肺叶切除手术较传统开胸手术创伤更小,术中出血少,术后恢复快,对细胞免疫功能影响较小。Objective： To investigate the function of the peroperative cell immune function in the non-small lung cancer underwent Video-assisted thoracoscopic surgery （ VATS ） and conventional thoracotomy.nethod： From Jan 2010 to Dec 2012, 60 patients with NSCLC were recuited and divided into two proups each content 30 patients ： V group （ VATS group ） and T group （ conventional surgery group ） . The operation duration, intraoperative blood loss and cell immune functions were analyzed. Result： （ 1 ） Compared with the T group, the tumor size, the duration of the operation time, the amount of bleeding blood transfusion and and hospitalization time were all less in the V group （ all P〈0.05 ） ; （ 2 ） There were no significant differences of CD4＋, CD8＋, CD4＋/CD8＋ between the two groups before the surgery （ P〈0.05 ） ; （ 3 ） The values of CD4＋, CD8＋ and CD4＋/CD8＋ in T group decreased significantly at one week after surgery compared with that before surgery （ all P〈0.05 ）, and in V group only CD4＋ decreased significantly after surgery （ P〈0.05 ） ; （ 4 ） At one week after surgery, The values of CD4＋, CD8＋ and CD4＋/CD8＋ in T group were all significantly lower than those in V group. （ all P〈0.05 ） .Conclusion： Compared with the conventional surgery, VATS can lead to less trauma, less bleeding, less impact on the peroperative cell immune functions and more rapid recovery.
