T型管治疗非肿瘤性胸段食管穿孔的疗效分析

摘　　要：目的通过比较非肿瘤性胸段食管穿孔的外科治疗方法,探讨T型管治疗该病的优势。方法整理本院50例非肿瘤性胸段食管穿孔患者的临床资料,分为T型管治疗(A组20例)、食管旷置后重建(B组15例)和食管切除后重建术(C组15例),比较分析不同方法治疗后患者的治疗效果。结果 A组患者全部治愈,无死亡病例,无二次手术,手术创伤小,无并发症,满意度高;B、C组患者无死亡病例,均需要进行二次手术,手术创伤较大,均出现并发症,临床患者满意度均较低(B组满意4例,C组5例),与A组(满意18例)相比,差异有统计学意义(P<0.05),且患者的治疗费用较高,差异有统计学意义(P<0.05),A组治疗时间为(8.0±1.3)周,B、C组均为(18.0±7.2)周,差异有统计学意义(P<0.05)。结论 T型管置入对非肿瘤性胸段食管穿孔患者的治疗效果显著,临床上可推广应用。Objective To discuss the superiority of T-shaped tube in the treatment of non-neoplastic thoracic esophageal perforation （NNTEP） by comparing several different surgical therapies. Methods The clinical data from 50 patients with NNTEP admitted into our hospital were collected and divided into three groups：group A （T-shaped tube therapy group, n=20）,group B （esophageal gullet reconstruction after indwelling, n=15）and group C （the reconstruction after the esophagectomy, n=15）.The therapeutic effects among the three groups were comparatively analyzed. Results In the group A,patients were all cured in small wounds without secondary surgery,and no death case occurred,even no complications,which displayed a high satisfaction.While in the group B and group C,there was no death case,but they were all underwent the secondary surgery with the large wounds and the complications occurred in all participants who displayed a low satisfaction. In the aspect of satisfaction,the number in the group A,B,and C was 18,4,and 5,which displayed statistical difference after comparing group A and group B or group A and group C （ P 0.05）.The cost of treat ment in the group much higher than that in the group A （P 0.05）.The treatment time in the group B or C lasted （18.0± 7.2） weeks,while in the group A,it only took （8.0±1.3） weeks,which demonstrated a statistical difference （P 0.05）. Conclusion In the treatment of NNTEP, applying the placement of T-shaped tube to treat the NNTEP obtains a remarkable effect, which is worthy of clinic promotion and application.
