正畸二次矫治患者主观矫治意识分析

摘　　要：目的调查正畸二次矫治患者主观矫治意识,为临床增进医患沟通、促进诊疗顺利实施提供帮助。方法收集2004-2011年第四军医大学口腔医院正畸科门诊二次矫治患者144例。其中男47例(32.6%),女97例(67.4%),平均年龄(19.0±7.4)岁。制定并发放问卷调查,调查内容涉及二次矫治患者首次矫治经历以及主观矫治意识调查。矫治意识包括矫治意愿、再次矫治期望、对自我牙列的认知、对自身错畸形矫治需求与错畸形的认知。患者首次与再次矫治意愿差异、对再次矫治期望应答差异采用卡方检验;对自我牙列的认知、自身错畸形矫治需要与错畸形的认知差异(视觉模拟评分VASs)采用变异系数分析。以P<0.05为差异有统计学意义。结果二次矫治患者首次与再次矫治意愿差异、再次矫治期望差异均有统计学意义(χ2=67.7414,P=0.0000;χ2=38.2146,P=0.0000);对自我牙列的认知、自身错畸形矫治需要与错畸形的认知差异(VASs)有统计学意义。结论二次矫治患者矫治意识普遍较高,正畸医师应加强理论与实践技能,增进医患沟通,了解患者所需,提高诊疗成功率。Objective The study was proposed to improve the clinical treatment based on the analysis of patient＇s self-perceptions in orthodontic re-treatment. Methods 144 patients （47 boys and 97 girls） with an average age of 19.0 years old who had an orthodontic treatment history were involved in this study. The questionnaires including people＇s decision, expectation, dental esthetic self-perception and malocclusion perception for re-treatment were given to the patients. Chi-square test was used to study on difference of patient＇s decision between first and second orthodontic treatment and patient＇s expectation for re-treatment. Subjects＇ differences in dental esthetic self-perception and malocclusion perception were assessed by multivariate analysis of variance. Results Significant differences were found in patient＇s decision between first and second orthodontic treatment and patient＇s expectation for re-treatment （X^2 = 67.7414, P = 0.0000;X^2 = 38.2146, P = 0.0000）. Multivariate analysis of variance also found the differences of the subjects in dental esthetic self-perception as well as self-perception of malocclusion. Conclusions Patients seeking re-treatment had increasing self-perceptions of orthodontics. Orthodontists should improve medical technology and provide better communication for patients.
