新生儿α-地中海贫血筛查及脐血红细胞参数变化分析

摘　　要：目的探讨脐血Hb Bart’s定量筛查α-地中海贫血(简称α-地贫)的价值及α-地贫新生儿脐血红细胞参数的变化。方法用Bayer ADVIA120全自动血细胞分析仪检测新生儿脐血红细胞数参数,Sebia全自动电泳仪对脐血进行Hb Bart’s定量检测,比较Hb Bart’s阳性组与阴性组脐血红细胞参数的差异。结果在1219份脐血样本中共有36份检出HbBart’s,其中静止型12例,标准型23例,中间型1例,未检出重型α-地贫,总检出率为2.95%。Hb Bart’s阳性组新生儿脐血RBC显著高于Hb Bart’s阴性组(P﹤0.01),而HB、MCV、MCH、HCT均显著低于Hb Bart’s阴性组(P﹤0.01)。结论脐血Hb Bart’s定量检测可有效筛查出新生儿α-地贫,脐血红细胞数参数可作为α-地贫的筛查指标。OBJECTIVE To investigate the value of quantitative analysis of Hb Bart’s in neonatal screening for α-tha-lassemia, and the changes of erythrocyte parameters in umbilical cord blood of newborn with α-thalassemia. METHODS Erythrocyte parameters in umbilical cord blood of newborn were determined by Bayer ADVIA 120 hematology analyzer system. Quantitative analysis of Hb Bart’s was performed by Sebia automatic electrophoresis apparatus. The values of erythrocyte parameters in umbilical cord blood of newborns with Hb Bart’s positive and negative were compared. RESULTS Out of the 1 219 cord blood samples, 36 were detected to have Hb Bart’s and the detection rate was 2.95%. Among the 36 case with Hb Bart’s positive, 12 cases were silent α-thalassemia, 23 cases were α-thalassemia minor and 1 case was α-thalassemia intermedia, no one case was α-thalassemia major. Newborns with Hb Bart’s positive had higher RBC value, but lower values of HB, MCV, MCH, HCT than newborns with Hb Bart’s negative. All the differences mentioned above had statistical sig-nificance （P﹤ 0.01）. CONCLUSION Quantitative analysis of Hb Bart’s in umbilical cord blood can be used to screen α-thalassemia effectively. Erythrocyte parameters in umbilical cord blood may be useful to screen α-thalassemia
