宫颈鳞癌患者血清鳞状上皮细胞癌抗原检测及其临床意义

摘　　要：目的　探讨宫颈鳞癌患者血清鳞状上皮细胞癌抗原 (SCCAg)的检测及其临床意义。方法　采用微粒子酶免疫分析法检测 12 4例宫颈鳞癌患者治疗前血清SCCAg值。早期患者 (Ⅰ～Ⅱa期 )术后 1周重复检测 ,部分手术患者随诊中监测。 结果　宫颈鳞癌血清SCCAg检测的敏感性为 73 .4% ,特异性为 96 .1%。SCCAg阳性率随临床分期进展而增高 ,有非常显著性差异 (P <0 .0 1) ,早期患者术前SCCAg异常者术后均下降至正常水平 ,有非常显著性差异 (Ⅰa期P <0 .0 1,Ⅰb +Ⅱa期P <0 .0 1)。部分术后患者随诊中血清SCCAg值在正常水平。 结论　SCCAg是宫颈鳞癌较特异的肿瘤标志物 ,可以作为宫颈癌辅助诊断指标 ,评价宫颈癌手术疗效 ,在早期宫颈鳞癌中的临床意义值得进一步研究。Objective To investigate the clinical implications of serum squamous cell carcinoma antigen(SCCAg) in patients with squamous cell carcinoma of the cervix.Methods The serum concentraction of SCCAg was determined by Microparticle Enzyme-Immunoassay(MEIA) in 124 patients with squamous cell carcinoma of the uterine cervix before treatment.The early-stage patients were remeasured one week after surgery.Partial patients after surgery were monitored for SCCAg regularly during follow-up.Results The abnormal SCCAg levels in diagnosis of squamous cell carcinoma of the cervix showed a sensitivity of 73.4% and a specificity of 96.1%.The SCCAg positive rates increased with elevation of clinical stage( P <0.01).The abnormal SCCAg levels of early-stage patients decreased to normal one week after surgery(Ⅰa P <0.01,Ⅰb+Ⅱa P <0.01) and it kept normal in some of them during follow-up.Conclusion SCCAg is a specific tumor marker of the squamous cell carcinoma of cervix.The SCCAg level elevates with clinical stages.The SCCAg can be used as a marker in adjuvant diagnosis of cervical cancer and to estimate the surgical efficacy.It is necessary to investigate the clinical significance of SCCAg in early stage of the cervix carcinoma.
