摘　　要：目的　探讨炎性乳腺癌的临床特点 ,治疗及预后因素。方法　对 48例炎性乳腺癌的临床资料进行回顾性分析 ,按TNM分期 ,Ⅲb期 38例 ,Ⅳ期 10例 ;无特定的组织学类型 ;48例患者均接受综合治疗。结果　全组病例平均生存期为 32个月 ,3年生存率为 45 .8% ,5年生存率为 33 .3 % ;Ⅲb期生存 5年者有 16例 ,Ⅲb期 1年内死亡者有 5例 ,Ⅳ期 10例中有 6例在 1年内死亡。结论　炎性乳腺癌的治疗主要为诱导化疗、手术、局部放疗及辅助化疗的综合治疗。其预后与乳房皮肤红斑范围、术前对化疗药物的敏感性、腋窝淋巴结转移、局部血管受累、临床分期等有关。Objective To study the clinical characteristics,treatment and prognosis of patients with inflammatory breast cancer(IBC).Methods Clinical data of 48 patients with IBC were reviewed retrospectively.According to TNM stage,there were 38 cases in stage Ⅲb,10 cases in stage Ⅳ.All patients received combination therapy.Results The median survival was 32 months.The 3-year,5-year survival rate was 45.8% and 33.3% respectively;16 cases in stage Ⅲb survived over 5 years,5 cases in stage Ⅲb and 6 cases in stage Ⅳ died within 1 year.Conclusion The prognosis of IBC is rather poor and related to the range of mammary erythema,sensitivity of preoperation chemotherapy,the axillary lymphnode metastasis and clinical pathologic stages.It suggests that the main treatment of IBC is combination therapy:guide chemotherapy and operation or local radiotherapy and assistant chemotherapy.
