摘  要:目的探讨乳腺癌瘤内动脉入射角度和瘤周动脉管径的变化特征。方法应用彩色多普勒超声、频谱多普勒超声及能量多普勒超声检测82例乳腺癌及30例纤维腺瘤瘤内动脉入射角度和瘤周动脉管径,并予以对比分析。结果 82例乳腺癌和30例纤维腺瘤周边、内部均可检测到动脉血供,乳腺癌组瘤内动脉入射角度(19.68°±9.54°)和瘤周动脉管径(2.57±0.26)mm分别<(P=0.000)和>(P=0.000)纤维腺瘤组(45.63°±6.62°)、(1.92±0.32)mm。以瘤内动脉入射角度≤32°和瘤周动脉管径≥2.4 mm提示乳腺癌其敏感性、特异性分别为87%、90%。结论乳腺癌具有瘤内动脉入射角度变小和瘤周动脉管径增宽的血供特点,它可为彩色多普勒和能量多普勒超声对乳腺良恶性肿瘤的鉴别诊断提供重要信息。Objective To detect the characteristic changes of incident angle of the plunging artery(IAPA) and the diameter of peripheral artery(DPA) of breast cancer. Methods 82 breast cancers and 30 fibroadenomas were evaluated on color Doppler and Spectrum Doppler and power Doppler ultrasonography, the incident angle of the plunging artery and the diameter of peripheral artery of breast tumors were measured and were analysed. Results 82 cancers and 30 fibroadenomas that had plunging arteries and peripheral artery were evaluated. The IAPA(19.68°± 9.54°) and the i)PA(2.57±0.26)mm of breast cancers were lower (P=0.000) and wider (P=0. 000) than that(45.63°±6.62°), (1.92±0.32)mm of fibroadenomas respectively, Using IAPA≤32°and DPA≥2.4 mm as the cutoff, the sensitivity and specificity in predicting breast cancer was 87% and 90% respectively. Conclusions There are the lower 1APA and wider DPA in breast cancer and they are one of the important values in the differentiation of breast tumors on color and power Doppler imaging.
