不同侧脐动脉缺失对胎儿先天畸形的影响

摘　　要：目的探讨在单胎妊娠中,单脐动脉胎儿合并其他系统畸形是否与脐动脉缺失侧的不同有关。方法对286例单胎妊娠的单脐动脉胎儿行系统扫查,其中144例合并其他系统畸形。产后随访到140例胎儿,其中134例产后证实为单脐动脉,6例为正常脐动脉胎儿。将134例产后证实的单脐动脉胎儿按脐动脉缺失侧的不同分为左侧脐动脉缺失组与右侧脐动脉缺失组。Fisher精确概率法分析两组胎儿合并不同系统畸形的发生率是否有差异。结果左侧脐动脉缺失组,合并单一及多发畸形的发生率分别为71.6%及28.4%。右侧脐动脉缺失组,合并单一及多发畸形的发生率为68.3%及31.7%。两组差异无统计学意义(P>0.05)。左侧脐动脉缺失组合并泌尿系统畸形的发生率为39.2%,合并肢体畸形的发生率为12.1%。右侧脐动脉缺失组中,合并泌尿系统畸形的发生率为23.3%,合并肢体畸形的发生率为5.0%。两者差异均有统计学意义(P<0.05)。结论在单胎妊娠中,左侧脐动脉缺失的单脐动脉胎儿更容易合并泌尿系统及肢体畸形。Objective To determine whether laterality of an absent umbilical artery（AUA） is associated with fe tal other system malformation in single umbilical artery （SUA）. Methods Of 286 fetuses with SUA, 144 cases had other system malformation. 134 cases were confirmed inpostpartum. 6 cases were 3 vessel cords. According to the lat- erality of an AUA, the fetuses with SUA were divided into the left AUA group and the right AUA group. Compare whether there is a difference in incidence in other system malformation between the two groups by Fisher＇s exact proba bility. Results In the left AUA group, the incidence in fetal combined with single and multiple malformations was 71.6~ and 28.4%, respectively. In the right AUA group, the incidence combined with single and multiple malforma tions were 68.3% and 31.7%. There were no statistically significant differences between the two groups （P：〉0.05）. The incidence of urinary system malformation was 39.2% and the incidence of limb deformities is 12. 1% in the left AUA group. The incidence of urinary system malformation is 23.3% and the incidence of limb deformities is 5.0% in the right AUA group. There were statistically significant differences between the two groups （P〈0.05）. Conclusions In a single pregnancy, the fetal with a left AUA has a higher incidence in urinary system and limb deformities.
