摘　　要：目的回顾性分析自身免疫性胰腺炎(AIP)患者的影像学资料,提高对AIP的早期识别。方法选取2003年1月～2011年10月该院外科收治的术后病理符合AIP亚洲标准的患者36例,总结AIP患者的影像学特点。结果 AIP影像学为胰腺弥漫性或局灶性肿大,胰头增大或肿块最常见,假包膜影具有特征性,钙化或胰石和囊性变可见,胰周渗出、索条影及淋巴结肿大相对少见,胰腺萎缩偶见,胰腺形态及密度无异常极少见,增强CT、彩色B超、内镜逆行胰胆管造影、磁共振胰胆管造影可见胰管串珠样扩张,胆总管狭窄常见。结论AIP作为一种特殊类型的慢性胰腺炎,具有特征性的影像学特点,应作为胰腺癌的一种鉴别诊断。[Objective ] To improve the early identification of autoimmune pancreatitis （AIP） by a retro- spective analysis of imageological data from AIP patients. [Methodsl The analysis included 36 patients ad- mitted by the surgery department of our hospital from January 2003 to October 2011 who had postoperative pathological findings consistent with Asian AlP standards. The imageologieal characteristics of these AlP pa- tients were analyzed. [Results] Imageology found a diffusely or focally enlarged pancreas, most frequently a mass or enlargement of the pancreatic head, characteristic pseudocapsule shadow, calcification or pancreatic calculus and cystic degeneration, but rarely found peripancreatic exudation, narrow shadow and enlarged lymph nodes, pancreatic atrophy, normal pancreatic shape and density, and string-of-beads dilation of pancreatic duct. Imageology frequently showed stenosis of the common bile duct. [ Conclusion] AIP as a unique type of chronic pancreatitis （CP） has distinctive imageologieal characteristics, which should be used for differentiation from pancreatic cancer.
