摘  要:目的探讨房水中VEGF-A和VEGF-C表达量与角膜新生血管和角膜新生淋巴管数变化规律,为角膜病患者选择合适的角膜移植手术时机提供理论依据。方法系统性回顾分析2011年4至10月期间因角膜病变在重庆医科大学附属第一医院眼科行穿透性角膜移植术的患者10例(10只眼)。采用管腔内皮非特异受体CD31和淋巴管内皮细胞特异性抗体LYVE-1行免疫组织化学染色,定量检测角膜新生血管数目(Blood vessels counting,BVC)和角膜新生淋巴管的数目(Lymphatic vessels counting,LVC)。将10个实验组样本和10个对照组(单纯年龄相关性白内障患者)的房水用酶联免疫吸附测定法(Enzyme linked immunosorbentassay,ELISA)对比试验,测出房水中VEGF-A和VEGF-C的表达量。结果采用配对t检验,Pearson相关检验进行统计学分析。结果免疫组化染色结果10例实验组角膜都有新生血管,4例既有新生血管又有新生淋巴管。实验组和对照组房水中VEGF-A和VEGF-C表达量差异有统计学意义,角膜新生血管、新生淋巴管与其房水的VEGF-A和VEGF-C的表达量有明显的关系。结论角膜病变后有角膜新生血管和新生淋巴管的患者,其房水中的VEGF-A和VEGF-C的表达量明显增高,且VEGF-A和角膜新生血管的数量呈正相关,VEGF-C和角膜新生淋巴管的数量也呈正相关,说明通过房水中VEGF-A和VEGF-C的检测可推知角膜新生血管和淋巴管的生长情况。Objective To evaluate the relationship between VEGF-A and VEGF-C expression in the aqueous humor with corneal neovascularization and lymphangiogenesis, to provide a theoretical basis for choosing the operation time of keratoplasty. Methods It was a retrospective case series study. Ten cases (10 eyes) of hospitalized patients suffering from keratopathy and requiting keratoplas- ty from April to October 2011 were analyzed retrospectively. Experimental group involved 10 pa- tients with keratoplasty following the corneal ulcer and alkali burn. Control group involved 10 pa- tients with simple age-related cataract. Blood vessels counting (BVC) and lymphatic vessels counting (LVC) were tested by nonspecific vessels endothelial antibody CD31 and immunohistochemical stain- ing of lymphatic vessel endothelial hyaluronic acid receptor-1 (LYVE-I). VEGF-A and VEGF-C con- centrations in the aqueous humor of patients and control group were measured by ELISA technolo- gy. The relationship between the expression of VEGF-A and VEGF-C in aqueous and the BVC and LVC was explored. Student t-test and Pearson correlation test were used to investigate the influenc- ing factor. Results Immunohistochemical staining showed that all of 10 patients in experimental group presented angiogenesis, of whom 4 presented both neovascularization and lymphangiogenesis. VEGF-A and VEGF-C expression in two groups were significant different. Corneal neovascularization and lymphangiogenesis were significantly related to VEGF-A and VEGF-C concentrations in the aque- ous humor of eyes. Conclusions VEGF-A and VEGF-C expression in the aqueous humor of eyes are significantly higher in patients with neovascularization and lymphangiogenesis following keratopa- thy than the control group. VEGF-A expression is positively related to the BVC. VEGF-C expres- sion is also positively related to the LVC. Therefore, VEGF-A and VEGF-C concentrations can re- flect the growth of corneal neovascularization and lymphangiogenesis.
