夜戴型角膜塑形镜对儿童对比度视力影响

摘　　要：目的研究夜戴型角膜塑形镜（Orthokeratology lens）对儿童对比度视力的影响及相关因素的分析。方法人组儿童28例，男性15例，女性13例，年龄为（10．25±1．60）岁，平均等效球镜为（-3．18±1．07）D，矫正视力≥1．0。使用多功能视力测试仪MFVA-100在戴镜前、戴镜1周及1月后测量四个对比度视力（100％，25％，10％，5％），以LogMAR形式记录。结果配戴角膜塑形镜前后，随着对比度的降低，LogMAR值均逐渐增大。在25％、10％、5％对比度下，戴镜1周（25％：t=-2．137，P=O．042；10％：t=-3．157，P=0．004；5％：t=-2．999，P=0．006，）及1月（25％：t=-2．322，P=O．028；10％：t=-2．958，P=0．006；5％：t=-2．989，P=0．006）后LogMAR视力值较戴镜前显著增大，差异有统计学意义。随着对比度的降低，戴镜前后LogMAR视力值差异越明显。配戴角膜塑形镜1月后LogMAR视力值与配戴角膜塑形镜1周后LogMAR视力值相比差异无统计学意义。对比度为5％时，戴镜1周（r=0．667，P〈0．0001）及戴镜1月（r=0．649，P〈0．0001）后LogMAR视力值改变量与初始近视度数存在显著相关性。结论夜戴型角膜塑形镜提高了戴镜后的裸眼视力，同时也降低了儿童的对比度视力，以低对比度下视力的降低更明显。Objective To evaluate the effect of overnight orthokeratology （ortho-k） lenses on the children＇s contrast visual acuity and analyze relevant factors. Methods A prospective study was conducted on 28 children （15 males and 13 females with mean age ＋SD of 10.25±1.60 years） wear- ing overnight ortho-k lenses. Mean spherical equivalent ＋SD was -3.1786±1.07337D and best-correct- ed visual acuity （VA） reached at least 20/20. Contrast visual acuity （100%, 25%, 10%, 5%）was de- termined by MFVA-100 before, one week and one month after commencement of the procedure. Results Either before or after wearing orthokeratology lenses, LogMAR values of VA gradually in- creased with the decrease of contrast. At contrast levels of 25%, 10% and 5%, there was a statisti- cally significant increase in the LogMAR value of VA after wearing orthokeratology lenses for one week （25%： P =0.042, t =-2.137; 10%： P =0.004, t =-3.157; 5%： P =0.006, t =-2.99） and one month （25%： P =0.028, t =-2.322; 10%： P =0.006, t =-2.958; 5%： P =0.006, t =-2.989）. The differ- ence was more obvious in lower contrast levels. There was no statistical difference between the Log- MAR value of VA after wearing orthokeratology lenses for one week and one month. At contrast level of 5%, the changes of LogMAR value of VA induced by orthokeratology were significantly cor- related with the amount of initial myopic diopter. Conclusions The overnight ortho-k lenses im- prove naked visual acuity of children as well as reduce their contrast visual acuity, specifically at the low contrast levels.
