玻璃体切除连续褥式缝合术治疗大范围睫状体脱离眼外伤

摘　　要：目的探讨玻璃体切除联合连续褥式缝合术在治疗伴有大范围睫状体脱离（离断口范围≥180°）复杂眼外伤的临床疗效。方法将36例经B超及UBM检查存在大范围睫状体脱离伴有玻璃体视网膜病变的复杂眼外伤患者先进行晶状体玻璃体切除，再由巩膜表面进针进入玻璃体腔内连续褥式缝合睫状体组织。观察术后视力、眼压及睫状体复位情况并进行统计分析。结果术后随访3-30个月，平均（12．0±4．7）个月，术后视力较术前有明显改善（P〈0．05），眼压较术前显著提高（P〈0．05）。睫状体复位成功率86．11％（31／36）。结论玻璃体切除联合连续褥式缝合术是治疗伴有大范围睫状体脱离（离断口范围≥180°）复杂眼外伤的安全有效手术方法之一。Objective To explore the clinical efficacy of vitrectomy combined with continuous mattress suture to treatment complicated ocular trauma with a wide range cyclodialysis （≥180°）. Methods Thirty-six ocular trauma patients with a wide range cyclodialysis accompanied vitreoretinopa- thy examined by B-ultrasound and UBM were included, after lens vitrectomy, continuous mattress su- ture ciliary body tissue through the scleral surface into the vitreous cavity. Observed postoperative vi- sual acuity, intraocular pressure, ciliary body reset and performed statistical analysis. Results The pa- tients were followed up for 3-30 months （average 12.0±4.7 months）. Postoperative visual acuity signif- icantly improved compared with the preoperative （P 〈0.05）, intraoeular pressure significantly improved compared with the preoperative （P 〈0.05）. Success rate of Ciliary body reset was 86.11%. Conclu- sions The extraction of vitrectomy combined with continuous mattress suture is one of safe and effec- tive surgical method to treatment complicated ocular trauma with wide range cyclodialysis （≥ 180°）
