摘 要:目的:研究负重与非负重条件下,不同种植体-基台连接对种植体颈部骨吸收及生物学宽度的影响。方法:6只犬随机编号,拔除下颌双侧第二、三、四前磨牙,3个月后植入Ankylos、GS-Ⅱ、US-Ⅱ种植体,据种植体-基台连接不同分A、G、U三组。第4个月后右侧行金属全冠修复,左侧不修复。种植即刻、第12个月拍X线片,测量种植体肩台(IS)-种植体-骨结合冠方最高点(BIC)距离。制作带种植体的骨磨片,图像分析定位:种植体周围龈缘(GM)、结合上皮根方(aJE)、BIC、IS,测量各组GM-aJE(ED)、aJE-BIC(CTC)和GM-BIC(BW)垂直距离。结果:在负重或非负重条件下,A、G组IS-BIC、ED、CTC与U组有差异,A、G组间无差异。3组BW均无差异。各组间负重与非负重条件下无差异。结论:平台转移设计可减少种植体颈部骨吸收,有更长结缔组织量。不同种植体-基台连接对生物学宽度影响不大,但对结合上皮附着位置有影响。Objective: To study peri-implant bone loss and biologic width around implants with different implant --abutment junction, under loading or unloading condition. Methods: Six adult dogs were numbered randomly, and their second, third, fourth premolars of mandibles were extracted. 36 implants were placed after three months. An- kylos,GS-Ⅱ[ ,US-Ⅱ were divided into 3 groups as Group A, Group G, Group U. Three months later, second stage surgery was performed. Implants on the right side were connected with metal crowns and functional loading after 4 months of 1st surgery, while implants on the left side were unloading at the same time. The animals were sacrificed after 12 months of 1st surgery. The distance between IS and BIC was measured. Sections with dental im- plant were made for histological analysis. The point of GM, aJE, BIC, IS was marked respectively. The distance of GM-- aJE(ED), aJE-- BIC(CTC), GM-- BIC(BW) was measured. Results : Under loading or unloading condition, the difference in IS--BIC, ED, CTC had statistical significance between group U and group A or group G, while there is no difference between group A and group G. The difference in BW had no statistical significance between group A, group G and group U. No matter they are loading or not, the self--comparison of each group presented no significant difference. Conclusion: After 12 months of 1st surgery, Implants with platform switching could re duce the peri--implant bone loss. Implants with different implant--abutment junction has little effect with biologic width, but it has an effect on the attached location of junctional epithelium, implants with platform switching show a higher length of connective tissue contact.
