摘  要:目的探讨06个月龄脑瘫高危儿血清神经元特异性烯醇化酶(neuron-specific enolase,NSE)表达特点及其在临床中的应用价值。方法通过ELISA法检测95例06个月龄脑瘫高危儿血清NSE浓度,采用Bayley婴幼儿发育量表(Bayley scalesof infant development,BSID)分析患儿智力和运动发育水平,统计学方法分析患儿血清NSE浓度与发育水平的相关性。结果(1)06个月龄脑瘫高危儿组血清NSE浓度显著高于与对照组(t'=19.44,P<0.01);高危因素组间血清NSE浓度不同,差异有统计学意义(F=14.12,P<0.01),其中以高胆红素血症组升高最为明显,与其他高危因素组比较差异有统计学意义(P<0.05),男性脑瘫高危儿血清NSE浓度升高水平较女性患儿明显(P<0.05)。(2)智力和运动发育迟滞组06个月龄脑瘫高危儿血清NSE浓度均高于临界组和正常组患儿,06个月龄脑瘫高危儿血清NSE浓度与智力发育指数和运动发育指数均呈负相关(MDI:r=-0.480,P<0.01;PDI:r=-0.463,P<0.01)。结论血清NSE能够反应患儿的脑损伤程度和发育水平,可以作为06个月龄脑瘫高危儿早期临床筛查的参考指标。Objective To investigate the characteristics and clinical significance of neuron - specific enolase in high - risk infants with cerebral palsy in 0 - 6 months. Methods The serum level of NSE was assayed in high - risk infants with cerebral palsy in 0 - 6 months by ELISA. The psychomoter development index (PDI) and mental development index(MDI) with BSID were detected. The correlation between the level of NSE and the development level are also determined. Results ( 1 ) The NSE levels of the high - risk infants with cerebral palsy in 0 - 6 months group were higher than the control group( t' = 19.44, P 〈 0.01 ). There were significant differences in NSE levels between different risk factors groups( F = 14.12,P 〈 0.01 ). The NSE level of hyperbilirubinemia group was the highest compared with other risk factors groups ( P 〈 0.05 ). The NSE level of male infants was higher than the female group ( P 〈 0.05 ). (2) The NSE levels of mental delay and psychomotor delay groups were higher compared with other risk factors groups. The NSE levels had negative correlation with MDI and PDI(MDI:r= -0.480,P〈0.01;PDI:r= -0.463,P〈0.01). Conclusion The serum level of NSE could reflect the severity extent of brain damage and the level of development in children. It can be used as a reference index for the early screening in high -risk infants with cerebral palsy in 0 -6 months.
