摘  要:目的建立基于化学发光微粒子免疫分析的乙肝表面抗原中和试验方法。方法在检测管中加入特异性抗-HBs,对照管中加乙肝5项标志物全阴性的血清。温育后,利用雅培ARCHITECT I2000化学发光仪进行HBsAg检测,依据公式计算抑制率。如果HBsAg≥0.05IU/ml,且抑制率≥50%,表示该标本被确认为HBsAg阳性。55例HBsAg弱反应血清样本分别用此方法和雅培Architect系统HBsAg确认试验进行了测试,并进行比对。结果结果显示55例样本全部被雅培HBsAg确认试剂确认为HBsAg阳性,而用笔者建立的方法被确认为HBsAg阳性的样本54例(98.2%),确认为阴性的样本1例(1.8%),其HB-sAg浓度为0.08IU/ml。对于HBsAg浓度≥0.10IU/ml的53例样本,两种方法全部确认为阳性。结论该方法可以对绝大多数HBsAg呈弱反应的标本进行确认,具有一定的应用价值。Objective To set up a neutralization assay of hepatitis B surface antigen based on chemiluminescent microparticle immunoassay. Methods Specific anti - HBS reagent was added into sample tube and sera which showed negative results of 5 hepatitis B markers was added into the control tube. After incubation,HBsAg was detected by Chemiluminescence Apparatus Abbott ARCHITECT I2000. Inhibition rate was calcalated according to the provided formulas. If HBsAg was ≥ 0.05IU/mL and the inhibition rate was≥ 50% ,it meant this sample was confirmed to be HBsAg positive. 55 serum samples which were detected for HBsAg as weak reactive were tested by our method and Abbott Architect System HBsAg Confirmatory assay for comparison. Results Results showed all 55 HBsAg positive sera were confirmed by Abbott HBsAg confirmatory assay reagent. Otherwise, 54 of these 55 cases (98.2%)were confirmed as HBsAg positive and 1 of them( 1.8% ) were confirmed as HBsAg negative by our method, whose HBsAg was 0.08IU/ml. All of the 53 samples whose HBsAg were ≥0.10IU/ml were confirmed as HBsAg positive by both reagent. Conclusion Most of the serum samples which were detected for HBsAg as weak reactive can be confirmed by this method, which is valuable in applications indeed.
