摘　　要：目的探讨胰岛素样生长因子ⅡmRNA结合蛋白3(IMP3)在宫颈上皮内瘤变及宫颈鳞癌中的表达水平及其意义。方法采用免疫组织化学方法检测558例宫颈组织标本IMP3表达情况,并对宫颈上皮内瘤变(CIN)Ⅱ、Ⅲ级患者进行随访观察。结果在无宫颈病变及CINⅠ组织中,IMP3均未见表达,CINⅡ、CINⅢ和宫颈鳞癌患者IMP3阳性表达率分别为1.82%、20.47%、95.74%,IMP3表达水平与宫颈病变程度呈正相关(r=0.502 3,P<0.000 1)。IMP3对宫颈鳞癌具有较高诊断价值,其灵敏度为95.70%,特异度为87.20%。随访结果表明仅在IMP3阳性表达的CINⅡ、CINⅢ级患者出现癌变,而IMP3阴性表达的CINⅡ、CINⅢ级患者均未进展为鳞癌。结论 IMP3在宫颈鳞癌的发生、发展过程中起重要作用,对于宫颈鳞癌有良好的诊断价值,且能作为预测宫颈上皮内瘤变患者进展为宫颈鳞癌的一个独立生物学标志物。Objective To discuss the expression and clinical significance of insulin-like growth factor Ⅱ mRNA binding protein 3（IMP3） cervical intraepithelial neoplasias and cervical squamous cell carcinoma.Methods The expressions of IMP3 in 558 samples of cervix were detected by immunohistochemical method.Patients with CINⅡ and CINⅢ were followed up for three years to observe the occurrence of cervical squamous cell carcinoma.Results Expression of IMP3 in cervical tissue without dysplasia and CINⅠwas negative.The positive expression rates of IMP3 in CINⅡ,CINⅢ and cervical squamous cell carcinoma were respectively 1.82%,20.47% and 95.74%.The expression of IMP3 was positively related to the degree of cervical lesion（r=0.502 3,P＆lt;0.000 1）.IMP3 had good diagnostic value to cervical squamous cell carcinoma with a sensitivity of 95.70% and a specificity of 87.20%.Invasive squamous cell carcinoma was found only in patients with IMP3-positive CINⅡ and Ⅲ,whereas no invasive squamous cell carcinoma was detected in patients with IMP3-negative CINⅡ and Ⅲ in their follow-up period.Conclusion IMP3 plays an important role in cervical squamous cell carcinoma and has high diagnostic value to cervical squamous cell carcinoma.IMP3 may be an independent biomarker to predict the progression of cervical intraepithelial neoplasias into invasive cancer.
