频域OCT分析急性中心性浆液性脉络膜视网膜病变形态学特征

摘　　要：目的频域相干光断层扫描(OCT)分析急性浆液性脉络膜视网膜病变(CSC)的视网膜形态特征。方法应用频域OCT对30例(60眼)单眼发病急性CSC患者的黄斑部视网膜检测,比较患侧眼组与对侧眼组黄斑区视网膜光感受器细胞全层厚度和视网膜素色上皮特征。结果 OCT示30例患者中患侧眼光感受器细胞全层厚度均匀一致12眼,对侧眼20例;患侧眼光感受器细胞全层厚度均匀一致伴内外节异常18眼(60.0%),对侧眼10眼(33.3%),差异有统计学意义(P=0.038)。患侧眼有24眼(80.0%)出现视网膜素色上皮(RPE)异常,对侧眼8眼(26.7%),差异有统计学意义(P=0.000)。结论频域OCT示单眼发病急性CSC患者部分对侧眼存在光感受器细胞层和RPE异常。Objective To observe the image morphology of acute central serous chorioretinopathy （ACSC） by using spectral domain optical coherence tomography （SD-OCT）. Methods The 30 patients （60 eyes） of unilateral resolved ACSC were selected in this study. The macular image of each case was obtained by SD-OCT. Compare the difference of photoreceptors and retinal pigment epithelium between the affected eyes group and the fellow eyes group. Results The thickness of photoreceptors were uniform in 12 eyes in affected eyes group, while 20 eyes in the fellow eyes. The thickness of photoreceptors were uniform but with abnormity in 18 eyes （60.0%） in affected eyes group, while 10 eyes （33.3%） in the fellow eyes. There exited statistically significant difference of the regularity of the photoreceptors be- tween the affected eyes group and the fellow eyes group （P = 0.038）. 24 eyes （80.0%） of the affected eyes group exited abnormity in retinal pigment epithelium morphology, while 8 eyes （26,7%） of the fellow eyes group. There was statisti- cally significant difference between the affected eyes group and the fellow eyes group （P= 0.000）. Conclusion The SD- OCT shows part of the fellow eyes of the unilateral resolved ACSC patient present abnormity in the photoreceptors lay- er and retinal pigment epithelium layer.
