人工鼻泪管植入术治疗慢性泪囊炎的有效性和安全性评价

摘　　要：目的评价人工鼻泪管植入术治疗慢性泪囊炎的有效性及安全性。方法选择2008年7月至2010年12月在我院行逆行性人工鼻泪管植入术治疗慢性泪囊炎的患者163例191眼,年龄26～73岁,平均57岁。所有患者术后随访2a,观察术后溢泪情况及泪道的通畅性。结果人工鼻泪管植入术后155例(182眼)患者溢泪症状完全缓解。随访期间,138例(166眼)患者人工鼻泪管保持通畅,25例(15眼)发生阻塞;随访过程中人工鼻泪管植入有效率为86.91%。术中有17例(17眼)患者出现疼痛,5例(5眼)出现眼睑水肿,所有患者均有少量鼻出血,均经对症处理后缓解。结论人工鼻泪管植入术是治疗慢性泪囊炎简单有效安全的方法,不愿意接受或不能耐受常规鼻腔泪囊吻合术的患者也能进行人工鼻泪管植入。Objective To probe into the effectiveness and safety of artificial naso-lacrymal duct implantation in treating chronic dacryocystisis. Methods One hundred and sixty-three patients （191 eyes） with chronic dacryocystisis that received retrograde artificial naso-lacrymal duct implantation from July, 2008 to December, 2010 were selected. They aged from 25 years old to 73 years old, and the mean was 57 years old. A 2-year follow-up was carried out. Epiphora condition and clearance of lacrymal passage were observed. Results Complete remission of epiphora was observed in 155 patients （182 eyes）. During the follow-up, artificial naso-lacrymal ducts in 138 patients （166 eyes） remained clear,obstruction occurred in 25 patients （15 eyes） ,the effective rate was 86. 91%. Intra-operative complications included aches in 17 patients （17 eyes） ; Palberal edema in 5 patients （5 eyes） ; Mild epistaxis in all patients. All the com- plications were alleviated after treatment. Conclusion Artificial naso-lacrymal duct implantation is a simple, effective and safe way in curing chronic dacryocystisis. It is suitable for patients who are not willing to receive or intolerant to traditional rhino-dacryocystostomy.
