摘  要:目的探讨常见血管危险因素(vascular risk factors,VRF)与阿尔茨海默病(Alzheimer’s disease,AD)的相关性。方法收集AD患者114例,以及与AD患者性别和文化程度匹配的认知功能正常的198名对照。收集年龄、性别、文化程度、病史、血压、空腹血糖、血脂、心电图和影像学资料等信息。分别采用单因素和多因素非条件Logistic回归及线性回归分析研究VRF与AD、认知功能评分的相关性。结果与对照组比较,AD患者年龄偏大[(74.46±8.87)vs.(69.03±8.68),P<0.001],患糖尿病(24.56%vs.13.13%,P=0.010)和冠心病(28.07%vs.10.61%,P<0.001)比例偏高。多因素Logistic回归分析显示糖尿病、冠心病和年龄与AD相关;多因素线性回归分析表明,年龄、冠心病、女性、糖尿病与MMSE评分呈线性关系。结论常见血管危险因素影响老年人的认知功能,与AD相关且可能促进AD的发生和发展。Objective To investigate the correlation between common vascular risk factors (VRF) and Alzheimer' s disease (AD). Methods One hundred fourteen AD patients from outpatient clinic and inpatient department of neurology were enrolled. One hundred ninety-eight age and education level matched cognitive normal controls were selected from health examination center. The information of age, sex, education level, medical history, fasting blood-glucose, blood lipid, electrocardiograph and image of CT or MRI scans was collected. Simple and multiple unconditional Logistic regression and linear regression were applied to study the correlations between VRF and AD and between VRF and MMSE scores. Results The age of AD patients was relatively older [(74.46±8.87) vs. (69.03±8.68),P〈0.001]. There were more patients with diabetes mellitus (24.56% vs. 13.13%, P=0.010) and corona'y heart disease (28.07% vs. 10.61%, P〈 0.001) in AD group than in controls. Multiple analysis indicated that diabetes mellitus, coronary heart disease and age were closely related to AD. Age, coronary heart disease, sex and diabetes mellitus were negatively correlated with MMSE scores. Conclusions Common VRF can affect the cognition function of the aged, which may contribute to the onset and progression of AD.
