摘 要:目的在肝脏局部流出道梗阻的基础上建立肝癌实验模型,观察肝脏出肝血流不畅时对肝癌细胞肝内播散的影响。方法16只雌性Buffalo大鼠分为两组,A组为对照组(SO),B组临时阻断肝脏左叶出肝静脉(OHV),并监测术中门静脉血流的变化。同时在两组大鼠肝内注入荧光标记的肝癌细胞。术后取下肝脏标本在荧光显微镜下观察两组模型肝内的肿瘤定植,常规观察流出道不畅后的肝脏病理变化。结果当大鼠肝脏左叶出肝静脉阻断后,入肝方向的门静脉血流由正常时的(11.00±3.13)ml/min下降为(8.42±1.99)ml/min。术后常规病理发现肝窦内大量淤血。荧光显微镜下观察,OHV组较s0组,肝内含有荧光标记的肿瘤明显增多(P〈0.05)。结论当肝脏流出道不畅造成肝内血流紊乱时,侵入脉管系统的肝癌细胞容易滞留于肝内,形成肝内转移灶。Objective To explore the effects of blood disturbance after hepatic venous occlusion procedures on intrahepatic metastasis of liver cancer cells in rats. Methods Sixteen female buffalo rats were randomly divided into occlusion of the hepatic vein (OHV) group and sham operation (SO) group. The por- tal flow was observed while the hepatic vein was clamped in the OHV group. After hepatic vein was clamped in the OHV group, the liver cancer cells labeled with the fluorescent dye were injected into the portal vein and the same with SO group. The animals were sacrificed at 2 h after the operation for the samples. DiI-la- beled liver cancer cells in the liver were then Observed under fluorescent microscopy. Histological analysis was taken to adjust the influence of hepatic outflow occlusion on liver environment. Results The OHV group showed an obvious decrease in portal flow, from ( 11.00 ±3.13) to (8.42 ± 1.99) ml/min after vein clam- ping. Pathological analysis also found numerous congestions around blood vessels and sinusoid and more Dil-labeled cancer cells in the livers of OHV group. Conclusion Occlusion of hepatic outflow may promote intrahepatic metastasis of cancer cells by enhancing the opportunity of stasis in the liver.
