高压氧同步联合放化疗治疗胶质瘤术后患者疗效观察

摘　　要：目的观察高压氧（HBO）同步联合放化疗治疗与单纯放化疗对胶质瘤术后患者的疗效、生存质量比较。方法治疗组为脑恶性胶质瘤32例经开颅显微手术后行高压氧联合放化疗治疗；对照组为脑恶性胶质瘤31例术后单纯行放化疗。结果行治疗组和对照组总有效率[完全缓解（CR）＋部分缓解（PR）]分别为93．75％、71．49％（P〈0．05）；两组Ⅱ、Ⅱ-Ⅲ、Ⅲ～Ⅳ级不同病理级别患者治疗后6个月ADL量表分值分别为：（18．18±3．63）、（22．89±5．71）分；（20．69±4．17）、（25．47±6．36）分及（26．20±7．36）、（36．86±8．21）分（P〈0．05），治疗组患者生存质量明显提高。结论显微手术后高压氧联合放化疗治疗脑恶性胶质瘤疗效较好。Objective To compare the short-term therapeutic effectiveness, survival duration and quality of life between patients receiving chemoradiotherapy combined with inhalation of hyperbaric oxygen therapy （HBO） vs. chemoradiotherapy alone in postoperative patients with malignant gliomas. Methods A total of 63 patients by initial operative treatment with malignant gliomas were analyzed. All patients were divided into two groups ： chemoradiotherapy combined with inhalation of HBO group （ n = 32） and chemora- diotherapy group （ n = 31 ）. Results The effective rate of total symptoms improved （ CR and PR） in the ehemoradiotherapy combined with inhalation of HBO group and chemoradiotherapy group was 93.75% and 71.49% respectively （P 〈0. 05）. Activities of daily living （ADL） scale of grades I1 , I] -I~, W-IV was assessed on all patients after the treatment. On the 6th month after treatment, ADL scales of grades Ⅱ , Ⅱ-Ⅲ and Ⅲ-Ⅳ in chemoradiotherapy combined with inhalation of HBO group and ehemoradiotherapy group were 18. 18 ± 3. 63, 22. 89 ± 5. 71 ; 20. 69 ± 4. 17, 25. 47 ±6. 36 ; and 26. 20 ± 7. 36, 36. 86 ± 8. 21 respectively （P 〈 0. 05 for all）, and the life quality of patients was improved obviously. Conclusion Mi- crosurgical resection combined with ehemoradiotherapy and inhalation of HBO is a safe and effective method for gliomas.
