摘 要:目的探讨基质金属蛋白酶-9(MMP-9)和E-钙粘蛋白(E-cadherin)在乳腺癌中的表达及其临床意义。方法采用免疫组化法检测20例正常乳腺组织、20例乳腺增生组织及50例乳腺癌组织中MMP-9和E-cadherin的表达,并分析两者与乳腺癌的临床病理指标之间的关系。结果 MMP-9在正常乳腺、乳腺增生及乳腺癌组织中的阳性表达率分别为5.0%、30.0%、66.0%,在乳腺癌组织中的表达显著高于正常乳腺和乳腺增生组织,而E-cadherin在三组中的阳性表达率则恰好相反,分别为95.0%、85.0%、42.0%,在乳腺癌组织中的表达显著低于正常乳腺和乳腺增生组织,两者在乳腺癌组织中的阳性表达率呈显著负相关(r=-0.605,P<0.05)。MMP-9和E-cadherin的表达均与乳腺癌组织学分级、肿瘤临床TNM分期、腋窝淋巴结转移与否显著相关(P<0.05),而与患者的年龄、肿瘤大小无显著相关性(P>0.05)。结论 MMP-9和E-cadherin均与乳腺癌的发生和发展密切相关,且二者在乳腺癌中的表达呈显著负相关,联合检测二者可能对乳腺癌的预后判断更具价值。Objective To study the expression and clinical significance of MMP-9 and E-cad_herin in breast cancer. Methods 20 cases of normal breast tissue from patients with fiberoadenoma, 20 cases of mammary hyperplasia tissues and 50 cases of breast invasive ductal carcinoma tissues were selected. The expressions of MMP-9 and E-cadherin were detected by immunohisto-chemistry, and the correlation of their expressions with the clinicpathological characteristics of patients with breast cancer was analyzed. Results The positive rate of MMP-9 expression in normal breast, breast hyperplasia and breast cancer tissues was re- spectively 5%, 30%, 66%, which was significantly higher in the breast cancer than in other tissues (P〈0.05). Whereas the posi-tive rate of E-cadherin expression in the three groups was respectively 95 %, 85 %, 42%, which was significantly lower in the breast cancer than that m other tissues (P〈0.05). There was a significant negative correlation between the expression of MMP-9 and E-cad-herin (r=0.605, P〈 0.05). Both the expressions of MMP-9 and E-cadherin protein were significandy correlated with the histological grade, tumorous TNM staging and axillary lymph node metastasis of breast cancer (P〈0.05), but not related to the age and tumor size (P〉 0.05). Conclusion Both MMP-9 and E-cadherin played importmxt roles in the developmertt of breast cancer, and their expressions were negatively correlated with each other in breast cancer. Therefore, the combination detection of the two could contribute to the prognosis evaluation of breast cancer.
