基质金属蛋白酶2的表达及其与胃癌临床生物学行为和预后的关系

摘　　要：目的探讨在胃癌组织、胃癌区域淋巴结及正常胃黏膜中基质金属蛋白酶2(matrix metalloproteinases-2,MMP-2)的表达情况,分析其与胃癌临床生物学行为、侵袭转移能力及预后的关系。方法选取行胃癌根治术并经病理证实的胃癌标本96例,胃癌区域淋巴结96例,正常胃黏膜60例,全部病例均随访5年以上。应用免疫组织化学方法检测上述标本中MMP-2的表达情况。结果胃癌组织中MMP-2的阳性表达率明显高于正常胃黏膜,并随分化程度的降低、浸润深度的加深表达增加,随临床分期的增加、淋巴结的转移而明显增强(P<0.05或P<0.01)。胃癌区域淋巴结组织中,有转移淋巴结组MMP-2的阳性表达率明显高于无转移淋巴结组(P<0.01)。胃癌患者生存期≥5年组MMP-2表达阳性率明显低于生存期<5年组(P<0.01)。结论 MMP-2的表达差异与胃癌的侵袭转移及预后关系密切。检测MMP-2对胃癌侵袭转移的预测、预后评估及指导合理的综合治疗等具有重要的临床意义。Objective To evaluate the expression of matrix metalloproteinase -2 （MMP-2） in human gastric cancer tissues, gastric regional lymph nodes and normal gastric mucosa tissues, analyze the relationship between its expression level and biological behavior, invasion and metastasis of human gastric cancer. Methods Sixty cases of normal gastric mucosa tissues, 96 cases of pathologically confirmed gastric cancer specimens and 96 cases of gastric regional lymph nodes were selected, and all patients were followed up for more than five years. The immunohistochemical method was used to detect the expression level of MMP-2 in different tissues. Results The positive expression rate of MMP-2 in gastric cancer tissues was significantly higher than that in normal gastric mueosa, and the expression was increased with the declined degree of differentiation and depth of invasion of tumors. The expression of MMP-2 was significantly enhanced with lymph node metastasis and clinical stage （ P 〈 0.05 or P 〈 0.01 ）. MMP-2 expression of regional lymph nodes of gastric cancer was significantly higher in metastatic lymph nodegroup than that of non-metastatic lymph node group （ P 〈 0.01 ）. The MMP-2 expression in gastric carcinoma tissue of patients with survival rate more than 5 years was significantly lower than those with survival rate less than 5 years （ P 〈 0.01 ）. Conclusion MMP-2 expression was closely related to the invasion, metastasis and prognosis of gastric cancer. Detection of MMP-2 has important clinical significance in the invasion and metastasis prediction, prognosis evaluation and guiding rational treatment of gastric cancer.
