单操作孔杂交式胸腔镜肺叶切除术的应用体会

摘　　要：目的:探讨单操作孔杂交式胸腔镜肺叶切除术的根治性、安全性及临床应用价值。方法:选取2008年10月至2011年12月54例临床早期非小细胞肺癌患者,其中30例行单操作孔杂交式胸腔镜肺叶切除术(杂交组),24例行全胸腔镜肺叶切除术(全胸腔镜组)。术前临床分期均为Ⅰ期,两组患者年龄、性别构成等临床资料差异无统计学意义。对比两组患者淋巴结清扫组数、手术时间、术中出血量、术后置管时间、术后住院时间、术后并发症发生率、术后第5天口述疼痛评分及住院费用等。结果:两组患者淋巴结清扫组数、手术时间、术中出血量、术后置管时间、术后住院时间、术后并发症发生率及术后第5天口述疼痛评分差异均无统计学意义(P>0.05);住院费用方面,杂交组低于全胸腔镜组(P<0.05)。结论:单操作孔杂交式胸腔镜肺叶切除术完全可达到全胸腔镜肺叶切除术的微创效果,手术难度较低,可操作性强,适于在基层医院推广应用;同时,此术式可节省一定的费用。Objective：To compare the radical effect,safety and clinical values between single-operation-port hybrid thoracoscopic and totally thoracoscopic pulmonary lobectomy.Methods：Fifty-four patients with early-stage non-small cell lung cancer underwent pulmonary lobectomy from Oct.2008 to Dec.2011.Single-operation-port hybrid thoracoscopic pulmonary lobectomy was conducted in 30 patients（hybrid group）,and totally thoracoscopic pulmonary lobectomy was conducted in 24 patients（totally thoracoscopic group）.All patients were identified as stageⅠbefore operation,and there were no differences in age and gender between the two groups.The number of dissected lymph nodes,operation duration,intraoperative blood loss,the time for postoperative catheterization,postoperative hospital stay,incidence of postoperative complications,the scores of pains dictated by the patients on the 5th day after the operation and hospitalization expenses of the two groups were compared.Results：There were no statistical differences in all the indexes mentioned above（P0.05） except the hospitalization expenses.The expense of hybrid group was less than that of totally thoracoscopic group（P0.05）.Conclusions：Single-operation-port hybrid thoracoscopic pulmonary lobectomy is as minimally invasive as totally thoracoscopic pulmonary lobectomy,and it is much easier and more inexpensive,so it is worthy of promotion in primary hospitals.
