摘　　要：目的探讨收缩压(systolic blood pressure,SBP)、舒张压(diastolic blood pressure,DBP)、脉压(PP)、平均动脉压(mean blood pressure,MAP)与脑卒中之间的关系以及收缩压与舒张压之间是否存在交互作用。方法对共58 530名1995-2005年间安庆及其周边地区流行病学研究的参与者进行回访,登记和确认脑卒中信息。用COX比例风险模型探讨不同的基线血压指标与脑卒中风险因素之间的关系。结果最终纳入分析的有21 533人,平均随访9.49年,共发生504例脑卒中。当人群血压水平维持在SBP≥100 mmHg、DBP≥75 mmHg、MAP≥85 mm-Hg或是PP≥40 mmHg的时候,脑卒中的发生风险与这些血压指标呈现正相关。没有观察到J型或者U型曲线。结论血压指标与脑卒中风险之间呈线性正相关,且血压对年轻组(35～55岁)发生脑卒中的影响更大。Objective To examine the relationships between BP index and stroke,and the interactions between systolic blood pressure（SBP） and diastolic blood pressure（DBP）.Methods From 1995 to 2005,several studies were conducted in districts of Anqing City and its surrounding areas.58 530 participants who took part in the baseline survey were contacted with in-home interviews,registered and confirmed of stroke information.Associations between stroke and each hazard ratios were determined using COX proportional hazards regression with adjustment for possible confounders.Results In this study,21 533 participants were evaluated.During an average follow-up of 9.49 years among survivors,504 strokes occurred.Higher SBP（SBP≥110 mmHg）,higher DBP（DBP≥75 mmHg）,higher MAP（MAP≥85 mmHg）,higher PP（PP≥40 mmHg） were associated with higher risk of stroke.Risk of stroke was in proportion to BP index.we found no U-or J-shaped relationships of blood pressure with stroke.Conclusions BP index was correlated linearly with the risk of stroke.There are greater benefits from larger BP reductions to reduce for stroke risk reduction,particularly in mid-aged people（35-55 years）.
