丁二酸洛沙平与利培酮对女性精神分裂症患者泌乳素水平变化的影响

摘　　要：目的探讨丁二酸洛沙平与利培酮对女性精神分裂症患者泌乳素水平变化的影响。方法将60例女性精神分裂症患者按随机数字表法分为丁二酸洛沙平组和利培酮组,每组30例。2组分别给予丁二酸洛沙平和利培酮治疗,4周为1个疗程。2组患者在治疗前,治疗2、4周进行阳性与阴性症状量表(PANSS)及副反应量表(TESS)评分;治疗前,治疗第1、2及4周进行血清泌乳素测定。结果丁二酸洛沙平组总有效率为60%,利培酮组总有效率为66%,2组比较差异无统计学意义(P>0.05)。治疗前2组PANSS总分及各因子分比较差异均无统计学意义(P>0.05);治疗后2、4周2组PANSS总分及各因子分均较治疗前明显下降(均P<0.05)。2组治疗前血清催乳素水平比较差异无统计学意义(P>0.05)。治疗2、4周后2组患者血清催乳素水平较治疗前均明显升高,利培酮组升高更显著(均P<0.05)。2组患者的不良反应均为轻度或中度,经对症处理后很快缓解。结论丁二酸洛沙平治疗女性精神分裂症与利培酮疗效相当,但对女性泌乳素水平影响较小。Objective To study the influence of butanediol loxapine and risperidone on prolactin levels in female patients with schizophrenia. Methods Sixty female patients with schizophrenia were randomly treated with butanediol loxapine（butanediol loxapine group, n=30） or risperidone（rispefidone group, n=30）for 4 weeks. Patients were assessed with Positive and Negative Syndrome Scale（PANSS） and Treatment Emergent Symptom Scale（TESS）before treatment and after treatment for 2 and 4 weeks. Serum prolaetin levels were measured before treatment and after treatment for 1,2 and 4 weeks. Results The total effective rate was 60% in butanediol loxapine group and 66% in risperidone group. The difference was not significant between the two groups （P〉0.05）. Moreover, there were significant differences in PANSS total and factor scores and serum prolactin levels between the two groups before treatment （P〉0.05）. After treatment for 2 and 4 weeks, PANSS total and factor scores decreased but serum prolactin levels increased in both groups （P〈0.05）. However, the increase in serum prolactin levels in risperidone group was more obvious than that in butanediol loxapine group （P〈0.05）. All adverse reactions were mild or moderate and were relieved after symptomatic treatment. Conclusion Butanediol loxapine is as effective as risperidone in the treatment of female schizophrenia. However, butanediol loxapine has less impact on prolactin levels than risperidone.
