乳腺癌HPV感染与P16蛋白失活的临床意义

摘　　要：目的探讨乳腺癌人乳头状瘤病毒(HPV)感染与P16蛋白失活之间的相关性,为乳腺癌预防治疗提供帮助。方法选取医院2009年3月-2012年3月行外科手术治疗乳腺癌患者74例及纤维腺瘤患者70例,分别采集乳腺癌组织、癌旁组织、纤维腺瘤组织及纤维腺瘤旁正常组织,进行HPVDNA分子原位杂交以及P16蛋白免疫组织化学检测,比较不同组织HPV感染阳性率,HPV感染与乳腺癌腋窝淋巴结转移相关性,P16蛋白免疫组化阳性率以及乳腺癌组织HPV感染阳性与P16蛋白表达相关性等。结果 HPV感染阳性率乳腺癌组织为83.8%、癌旁组织为54.1%、纤维腺瘤组织为18.6%及纤维腺瘤旁正常组织为10.0%,组间比较差异有统计学意义(P<0.05);HPV感染阳性率与乳腺癌腋窝淋巴结转移呈正相关关系,组间比较差异有统计学意义(P<0.05);乳腺癌组织P16蛋白免疫组化阳性率为29.7%,明显低于癌旁组织的51.4%、纤维腺瘤组织的68.6%及纤维腺瘤旁正常组织的81.4%,组间比较差异有统计学意义(P<0.05);而乳腺癌组织HPV感染阳性率与P16蛋白表达呈负相关关系,组间比较差异有统计学意义(P<0.05)。结论乳腺癌组织中HPV感染率高,且与抑癌基因P16蛋白表达呈负相关关系,其感染与否亦可作为评价乳腺癌预后的重要指标。OBJECTIVE To investigate the correlation between expression of HPV and P16protein in the patients with breast cancer so as to provide basis for the prevention and treatment of breast cancer.METHODS A total of 74patients with breast cancer and 70patients with breast fibroadenoma were chosen in the period from Mar 2009 to Mar 2012,then the breast cancer tissue,tissue adjacent breast cancer,fibroadenoma tissue,and normal tissue adjacent fibroadenoma were collected,and the positive rate of HPV infection in different tissues,the correlation of HPV infection with axillary lymph node metastasis,the positive rate of P16protein immuno-histochemistry in different tissues,and the correlation between expression of HPV and P16protein in breast cancer were compared.RESULTS The positive rate of HPV infection（83.8%）in breast cancer tissue was significantly higher than that in the tissue adjacent breast cancer（54.1%）,the fibroadenoma tissue（18.6%）or the normal tissue adjacent fibroadenoma（10.0%）,the difference was statistically significant（P〈0.05）.The positive rate of HPV infection was positively correlated with the axillary lymph node metastasis（P〈0.05）.The positive rate of P16protein immunohistochemistry in the breast cancer tissue（29.7%）was significantly lower than that of the tissue adjacent breast cancer（51.4%）,the fibroadenoma tissue（68.6%）or the normal tissue adjacent fibroadenoma（81.4%）,the difference was statistically significant（P〈0.05）,however,there was a negative correlation between the expression of HPV and P16protein in the breast cancer patients（P〈0.05）.CONCLUSION The positive rate of HPV infection in the breast cancer tissues is high,which is negatively correlated with the expression of P16 protein.It can be defined as a significant indicator for evaluating the prognosis of breast cancer.
