摘  要:目的在双人袖套法建立小鼠颈部心脏移植模型的基础上,对其核心步骤进行改进,使手术由单人即可完成。方法供、受体分别为BALB/c小鼠(50只)和C57BL/6小鼠(50只),应用内芯固定套管的"袖套"法血管吻合技术,进行小鼠颈部心脏移植。术后每天触摸小鼠颈部观察移植心脏的搏动情况。移植心脏搏动持续3d以上即表示移植成功,观察移植心脏的存活时间。术后第7天随机选取10只受体小鼠将其处死,取其移植心脏,经10%多聚甲醛固定、石蜡包埋、切片后脱水、固定,经苏木精-伊红(H-E)染色,置于光学显微镜下观察移植心脏的心肌结构,判断受体小鼠移植心脏的排斥反应分级。结果正式实验50次。术后移植心脏复跳率为100%。1只受体小鼠术后未苏醒,另1只受体小鼠术后第2天移植心脏停跳;单人袖套法建立小鼠颈部心脏移植模型的成功率为96%(48/50)。供心缺血时间18~35min,平均供心缺血时间为(26.2±5.4)min;手术时间73~119min,平均手术时间为(93.5±12.7)min;移植心脏存活时间7~9d,平均移植心脏存活时间为(8.1±0.6)d。病理学检查示,移植心脏的心肌细胞间质有大量淋巴、巨噬细胞浸润,伴有明显的水肿、出血和心肌坏死。10只小鼠的移植心脏均出现排斥反应,其中3A级排斥反应2只,3B级5只,4级3只。结论经改进后的单人袖套法建立小鼠颈部心脏移植模型的成功率较高,且操作简便,值得推广。Objective To establish a stable cervical heterotopic heart transplantation modal in mice based on the cuff technique so that operations can be done by a single person. Methods The donors and acceptors were BALB/c (n = 50) and 057BL/6 (n = 50) mice, respectively. The cuff vascular anastomosis technique with a sleeve fixed at the core was used for cervical heterotopic heart transplantation in mice. The neck was touched every day after the operation to observe the pulsation of the transplanted heart. The transplantation was thought to be successful once heart beat lasted for more than 3 consecutive days. The survival time of the transplanted hearts were observed. Ten randomly-selected acceptors were killed on Day 7 after operation, and their transplanted hearts were taken and subjected to 10% polyoxymethylene fixation, paraffin embedding, post- slicing dehydration and fixation, and hematoxylin and eosin (H-E) staining. Tissues from the transplanted hearts were observed for myocardial structure and the rejection of each transplanted heart was judged. Results The experiment was conducted for 50 times, and the graft cardiac rebeating was 100%. One acceptor did not revive after the operation, and the transplanted heart of another acceptor stopped beating on Day 2 after the operation. The success rate of modeling for mice using the single-person cuff technique was 96% (48/50). The average duration of myocardial ischemia of donors was (26.2±5.4) min (range, 18 to 35 min). The average operation time was (93.5± 12.7) min (range, 73 to119 min). The average survival time of the transplanted hearts was (8.1±0.6) d (range, 7 to 9 d). Under optical microscope, substantial lymphocyte and macrophage infiltration were found in the myocardial intercellular substance of the transplanted hearts, accompanied by apparent edema, hemorrhage and myocardial necrosis.
