腹腔镜手术治疗子宫恶性肿瘤65例临床分析

摘　　要：目的探讨腹腔镜手术治疗子宫恶性肿瘤的临床疗效、安全性及并发症。方法收集该科2007年5月～2012年8月腹腔镜手术治疗子宫颈癌和子宫内膜癌的65例患者,对其临床资料及近期疗效进行回顾性分析。结果 65例患者手术时间(281±53.6)min(160～390min),手术出血量(327.8±235.5)mL(80～1700mL)。术中并发症有膀胱损伤1例,髂外血管损伤1例,腰大肌血管撕脱1例,中转开腹2例;术后并发症有淋巴囊肿8例,尿潴留7例,阴道残端感染2例,膀胱阴道瘘2例,术后体温大于38℃持续2d8例,下肢静脉血栓1例。术后近期随访无1例复发。结论腹腔镜手术治疗子宫恶性肿瘤是安全、可行的,可以作为子宫恶性肿瘤的手术方式之一。[Objective] To investigate the clinical effect, security, and complication of uterine malignancy operated by laparoscopic surgery. [Methods] A retrospective analysis for the cases with uterine malignancy operated by laparoscopy in Hunan Province Tumor Hospital from May 2007 to August 2012 were conducted. The clinical data and follow-up results were also reviewed. [Results] The mean operative time and blood loss were （281±53.6） min （160～390 min） vs. （327.8±235.5） mL （80～1700 mL）. The intraoperative complications included： 1 case of bladder injury, 1 case of the iliac artery injury, 1 case of psoas major vascular avulsion, 2 cases of conversion to open laparotomy; The postoperative complications included： 8 cases of lymphocyst, 7 cases of urinary retention, 2 cases of vaginal-cuff infection, 2 cases of uretero-vaginal fistula, 8 cases of postoperative body temperature more than 38℃ lasting 2 days, 1 case of deep venous thrombosis without case of recurrence in the recent follow-up. [Conclusions] Laparoscopic surgery for uterine malignancy is safe, better recent term effect. It is one of the selective methods for treatment of uterine malignancy.
