肾盂尿路上皮癌CD44和CD8表达生物学意义的探讨

摘　　要：目的:探讨CD44蛋白和CD8+T细胞在肾盂癌组织中的表达及临床意义。方法:应用免疫组织化学SP法检测76例不同级别肾盂癌组织中CD44蛋白和CD8+T细胞的表达,分析其在不同临床病理因素肾盂癌组织中的表达,及两者在肾盂癌发展过程中的相互关系。结果:肾盂癌组织中,CD44弱表达率为36.8%,中等强度表达率为32.9%,高表达率为30.3%,CD44在肌层浸润性(pT2、pT3或pT4)和高分级肾盂癌组织中表达下调,CD44表达与肿瘤分期和分化程度均呈负相关,r值分别为-0.547和-0.365,P值分别为0.003和0.001;CD8+T细胞低表达率为34.2%,中等强度表达率为34.2%,高表达率为31.6%,CD8+T细胞与肿瘤分期和分化程度均呈负相关(r=-0.683,P=0.000和r=-0.308,P=0.006),且肾盂癌组织中CD8+T细胞的表达与CD44的表达呈正相关,r=0.372,P=0.001。结论:CD44和CD8+T细胞与肾盂癌的分化、浸润等恶性行为有关,同步检测两者在肾盂癌组织中的表达并综合分析两者的关系,对评价肾盂癌的侵袭转移能力判断具有一定价值。OBJECTIVE： To investigate the expression of CD44 and CD8＊ T cell and their clinical significance in urothelial carcinoma of renal pelvis. METHODS： CD44 and CD8 were analyzed by immunohistochemistry using a mono- clonal antibody CD44 and CD8 in 76 cases of urothelial carcinoma of renal pelvis. The levels of CD44 and CD8 expression were analyzed in different clinical pathological factors. Moreover,CD44 expression was investigated in comparison with CDS. RESULTS： The weak（ ＋ ）, moderate（ ＋＋ ） and high expression（ ＋＋＋ ） rate of CD44 was 36. 8%, 32. 9% and 30.3 % respectively. The expression of CD44 decreased with increased clinical stages, pathological grades, and there was negative correlation between expression of CD44 and different clinical stages, different pathological grades in urothelial carcinoma of renal pelvis （ r= 0. 547, P = 0. 003 or r = -- 0. 365, P = 0. 001, respectively）. The weak （ ＋ ）, moderate （ ＋＋ ） and high expression （ ＋＋＋ ） rate of CD8 was 34.2 %, 34.2% and 31.6 % respectively. The expression of CD8 ＋ T cell decreased with increased clinical stages, pathological grades, and there was negative correlation between expression of CD8＋T cell and different clinical stages, different pathological grades in urothelial carcinoma of renal pelvis （r= 0. 683, P= 0. 000 and r= --0. 308, P= 0. 006, respectively）. Meanwhile, there was a positive correlation between the expression of CD44 and that of CD8＋T cell （r=--0. 372,P=0. 006）. CONCLUSIONS.. The expression of CD44 and CD8 T cell is as- sociated with tumor malignancies, such as differentiation and invasion. The evaluation of the two markers will be applicable guidance for the diagnosis of metastasis and invasion of urothelial carcinoma of renal pelvis.
