摘  要:背景:慢性胰腺炎(CP)病程不可逆,骨髓间充质干细胞(MSCs)具有定向迁移、损伤修复的功能,可能参与CP病程。目的:探讨骨髓干预对L-精氨酸诱导CP病情的影响。方法:将大鼠分为6组,三组以L-精氨酸制备CP模型,其中两组分别以白消安和G-CSF预处理诱导骨髓抑制和骨髓动员。其余三组分别腹腔注射白消安、G-CSF和0.9%NaCl溶液。模型制备结束后处死大鼠,检测血清淀粉酶水平,行胰腺组织病理学评分以及血液涂片和骨髓涂片的Wright-Giemsa染色,流式细胞术测定MSCs细胞CD29+CD44+CD45-细胞比例。结果:骨髓抑制-CP大鼠7 d生存率明显低于骨髓动员-CP大鼠和CP大鼠(50%对100%、100%,P<0.01)。三组CP大鼠病理学评分显著高于相应非CP大鼠(P<0.01),而血清淀粉酶水平无明显差异。骨髓抑制组、骨髓抑制-CP组外周血和骨髓有核细胞数量显著减少,骨髓动员组和骨髓动员-CP组外周血和骨髓有核细胞数量明显增多。骨髓抑制组CD29+CD44+CD45-细胞比例显著低于骨髓动员组和正常对照组(3.04%±0.89%对11.49%±1.81%、6.45%±1.26%,P<0.05)。骨髓抑制-CP组MSCs细胞比例显著低于骨髓动员-CP组和CP组(1.83%±0.66%对10.93%±1.74%,7.06%±1.60%,P<0.01)。结论:MSCs功能状态与CP病情相关,MSCs在CP时发挥保护作用,促进损伤修复。Background: Chronic pancreatitis(CP) has an irreversible course.Bone marrow mesenchymal stem cells(MSCs) may take a part in the course of CP with the ability of orientation migration and injury repair.Aims: To investigate the influence of bone marrow intervention on L-arginine-induced CP model.Methods: Rat were divided into six groups.CP model was induced by L-arginine in three groups,and busulfan or G-CSF was preadministered to induce bone marrow suppression or bone marrow mobilization.Rats in the other three groups were intraperitoneally injected with busulfan,GCSF and 0.9% NaCl solution,respectively.Rats were sacrificed after the establishment of model.Serum amylase was determined,pathological score of pancreatic tissue,Wright-Giemsa staining of blood and bone marrow slides were performed,and ratio of CD29 + CD44 + CD45-cells was detected by flow cytometry.Results: 7-day survival rate in bone marrow suppression-CP group was significantly lower than that in bone marrow mobilization-CP group and CP group(50% vs.100%,100%,P〈0.01).Pathological score in three CP groups was significantly higher than that in corresponding non-CP groups(P〈0.01),and no significant difference in serum amylase level was found between three CP groups and corresponding non-CP groups.There was a significant reduction in the number of nucleated cells in peripheral blood and bone marrow in bone marrow suppression group and bone marrow suppression-CP group,but the number of nucleated cells was significantly increased in bone marrow mobilization group and bone marrow mobilization-CP group.Ratio of CD29 + CD44 + CD45-was significantly decreased in bone marrow suppression group than in bone marrow mobilization group and control group(3.04% ± 0.89% vs.11.49% ± 1.81%,6.45% ± 1.26%,P〈0.05),ratio of MSCs was significantly decreased in bone marrow suppression-CP group than in bone marrow mobilization-CP group and CP group(1.83% ± 0.66% vs.10.93% ± 1.74% and 7.06% ± 1.60%,P〈0.01).Conclusions: The
