白内障3mm小切口手法碎核乳化摘出的临床应用

摘　　要：目的探讨表面麻醉下3．0mm带球结膜蒂角膜缘隧道式切口白内障手法碎核乳化摘出联合折叠人工晶状体植入术的手术方法和应用效果。方法对50例（50眼）的Ⅰ-Ⅲ级核的白内障在表面麻醉下行3．0mm带球结膜蒂角膜缘隧道式切口，经劈核、分核、捣核、搅核等碎核乳化后摘出并植入折叠式人工晶状体。术后不包眼，观察手术并发症及术后视力变化。结果术后6h视力≥0．3者为26眼（52％）；术后1d视力≥O．5者41眼（82％）；术后1周视力I〉0．5者46眼（92％）；术后1月视力t〉0．5者48眼（96％）。无严重术中及术后并发症。结论对Ⅰ-Ⅲ级核白内障手法碎核乳化摘出折叠人工晶状体植入术，是一种较好而实用手术方式。Objective To explore the surgical approach and effectiveness of the cataract extraction after manual fragmentation and emulsification of nucleus with 3.0 mm tunnel limbus incision through predic- ted bulbar conjunctiva after surface anesthesia, integrated with the implantation of preloaded foldable artifi- cial intraocular lens. Methods After surface anesthesia, a 3.0 mm tunnel limbus incision was operated through the predicted bulbar conjunctiva and sclera on 50 eyes of 50 cases with Ⅰ-Ⅲ nuclear cataract. And after the fragmentation and emulsification of nucleus which consists of splitting, separating, smashing, and disturbing of the nucleus, the preloaded foldable intraocular lens was implanted. After the operation, without enswathement, the eye was monitored for postoperative complications and variation of eyesight. Re- suits 6 hours after the operation, 26 eyes had their eyesight ≥0.3, taking up 52% of the whole; one day after operation, 41 eyes had their eyesight ≥0.5, taking up 82% of the whole; one week after operation, 46 eyes had their visual acuity ≥0.5, taking up 92% of the whole; one month after operation, 48 eyes had their visual acuity ≥0.5, taking up 96% of the whole. There was no complication during-operation or post- operatively. Conclusion Manual fragmentation and emulsification of nucleus with foldable intraocular lens implantation is a fairly ideal and practical surgery.
