经直肠13针穿刺活检和血清PSA浓度诊断前列腺癌的相关性分析

摘　　要：目的探讨超声引导下经直肠前列腺13针穿刺病理结果与血清前列腺特异性抗原(prostate specific antigen,PSA)含量之间的关系。方法回顾性分析122例经直肠超声引导下13针穿刺法活检的患者,评价前列腺癌的检出率,观察血清PSA浓度与前列腺癌之间的关系。结果 PSA<4.0ng/mL、4.0≤PSA<10ng/mL、10≤PSA<20ng/mL、≥20ng/mL各个水平中阳性率分别为12.6%、23.8%、40.7%、77.4%,四组间阳性率的比较均有明显差异(P<0.05)。结论血清PSA浓度与前列腺癌穿刺活检阳性结果密切相关,血清PSA浓度对前列腺癌穿刺活检的实施有一定指导意义。Objective To assess the correlation of transrectal ultrasound prostate biopsy of 13-core and the serum concentration of prostate specific antigen （PSA） in diagnosis of prostate cancer. Methods 105 patients treated with the transrectal ultrasound prostate biopsy of 13-core were retrospectively analyzed, and their detection rates of prostate cancer were assessed. The correlation between the concentration of PSA and prostate cancer were analyzed. Results The positive rate of prostate cancer with PSA level at 〈4.0 ng/mL was 12.6%, 4.0≤PSA 〈10 ng/mL was 23.8%, 10≤PSA 〈20 ng/mL was 12.6%, and ≥20 ng/mL was 12.6%, respectively. The positive rate among those four groups were significantly different （P 〈0.05）. Conclusions The serum concentration of PSA and biopsy-positive results for prostate cancer are significantly associated. The serum concentration of PSA is a useful marker to guide the implementation of transrectal ultrasound prostate biopsy in the diagnosis of prostate cancer.
