对主观全面评估法在消化道肿瘤患者营养评估及预后预测价值的认识

摘　　要：目的证明主观全面评估法在中国消化道肿瘤患者营养评估及预后预测的价值。方法选取2008～2010年486例初步诊断为消化道肿瘤并需手术治疗的患者,男262例(53.9%)、女224例(46.1%)。术前应用SGA(Subject Global Assessment主观全面评估法)评估每位患者的营养状态,同时测量人体参数及相关的实验室检查,包括前白蛋白(PA)、白蛋白(ALB)的水平。术后评估包括:住院时间、术后并发症的发病率、住院费用。结果根据术前SGA的评估结果将病人分为3组:营养正常组(A组)、轻-中度营养不良组(B组)、重度营养不良组(C组)。每组分别为256例(52.7%)、202例(41.6%)、28例(5.7%)。单因素方差分析结果表明各组间在体重指数(BMI),肱三头肌皱褶的厚度(TSF)、前白蛋白(PA)、白蛋白(ALB)、住院时间、住院费用间的差异有统计学意义(P﹤0.05)。并发症的发生率各组间的差异无统计学意义(χ2=4.16,P﹥0.05)。结论 SGA是一个安全、无创、简单的评估中国消化道恶性肿瘤患者营养状态的可靠方法。SGA的指标可以预测某些特定的疾病转归:如住院时间、住院费用。OBJECTIVE To prove the value of subject global assessment’s effects on nutrition assessment and prognostic value of Chinese gastrointestinal cancer patients. METHODS 486 gastrointestinal cancer patients of primary diagnosis and surgical treatment from 2008 to 2010 were elected. 262 cases （53.9%） were males and 224 cases （46.1%） were females. Every patient was evaluated with Subject Global Assessment （SGA） for his / her nutritional status, at the same time, related parameters measurement and laboratory tests, including Prealbumin （PA） , Albumin （ALB） level on preoperation were detected. Hospital stays, incidence of postoperative complications, hospital costs on post-operation were collected. RESULTS According to the evaluation results all patients were divided into three groups： normal nutrition group （group A） with 256 cases （ 52.7% ） , light-moderate malnutrition group （ group B） with 202 cases （ 41.6% ） and severe malnutrition group （group C）with 28 cases （5.7%）. Single factor analysis of variance results showed that it had a statistical difference on Body Mass Index （BMI）, Triceps Skin Fold （TSF）, PA, ALB, Hospital stays, Hospital costs （P 0.05）. There was no statistical difference on incidence of complications between groups （χ2 = 4.16, P﹥0.05）. CONCLUSION SGA is a safe, non-invasive, reliable, simple and convenient evaluative method for nutrition states of chinese gastrointestinal cancer patient. Its index could predict prognosis for certain disease, such as hospital stays and hospital costs.
