摘  要:目的:比较两种常用的人乳头瘤病毒(HPV)的检测方法一致性。方法:将南方医科大学第三附属医院与广州暨南大学附属第一医院的1080例妇女生殖道宫颈脱落细胞标本按组织病理结果分为宫颈炎组,宫颈上皮内瘤变(CIN)Ⅰ组、Ⅱ组、Ⅲ组和湿疣组。采用实时荧光聚合酶链反应技术(Real-time PCR)与酶切信号放大法(Invader Technology)分别检测这些标本中的高危性HPV,比较两种不同方法各组检测结果的一致性。结果:Real-time PCR和酶切信号放大法两种方法检测高危型HPV的一致性极好,总符合率为96.76%,总Kappa指数(KI)为0.934,宫颈炎组KI为0.908,CINⅠ、Ⅱ、Ⅲ组KI分别为0.872、0.917、0.758,湿疣组KI为0.934。结论:Real-time PCR和酶切信号放大法两种检验方法具有极好的一致性,都是检测HPV感染的有效手段。Objective: To compare the consistency of two commonly used tests for Human Papillomavirus (HPV). Methods: One thousand and eighty cervical exfoliated cell samples from the Third Affiliated Hospital of Southern Medical University and the First Affiliated Hospital of Jinan University were classified into five groups according to their pathological diagnosis : chronic cervicitis, CIN I , CIN II, CIN III and condyloma acuminatum. Real -time polymerase chain reaction( Real- time PCR) and invader technology were used for detection of high -risk HPV in these samples, respectively. The accordance of the results was compared. Results : The total accordant rate of real - time PCR and invader technology in high - risk HPV detection was 96.76% and general Kappa Index was 0. 934. The Kappa indices of chronic cervicitis, CIN I , CIN II , CIN III and condyloma acuminatum group were 0. 908,0. 872,0.917,0. 758 and 0. 934, respectively. The accordance was excellent. Conclusion: Real - time PCR has excellent accordance with invader technology in high - risk HPV detection. Both real - time PCR and invader technology are effective methods to detect high - risk types of HPV infection in cervix.
