摘　　要：目的:本文旨在通过对比LigaSure与传统分离结扎在门静脉高压症脾切除、贲门周围血管离断术中的应用,来证实LigaSure是否同样适用于门静脉高压症手术。方法:将拟行脾切除、贲门周围血管离断术的患者共100例,随机分成LigaSure组(50例)和传统分离结扎组(50例),两组患者在肝脏原发疾病、肝功能、食管静脉曲张程度方面无差异(P<0.05);比较手术耗时、术中出血量,术后腹腔引流量、脾热、门静脉血栓、住院天数等。结果:两组在术后腹腔引流量、脾热、门静脉血栓、住院天数方面无差异,而手术耗时、术中出血差异显著(P<0.001)。结论:LigaSure在门静脉高压症患者的脾切除、贲门周围血管离断术的应用是安全、有效的,能显著的缩短手术时间和减少出血量,而术后并发症与传统分离结扎手术无差别。Objective： This paper aims at separating ligation comparison between LigaSure and traditional methods in portal hy- pertension splenectomy, pericardial paraesophsgastric devascularization of application, to confirm whether LigaSure is also suitable for operation in patients with portal hypertension. Methods： Undergoing splenectomy, pericardial paraesophsgastric devascularization in pa- tients with a total of 100 cases, were randomly divided into LigaSure group （50 cases） and the traditional separation of ligation group （50 cases）, two patients had no difference in the primary liver disease, liver function, the degree of esophageal varices; observe the operation time, intraoperative bleeding volume, postoperative drainage volume, spleen heat, portal vein thrombosis, days in hospital. Results： There was no significant difference between two groups that were in the postoperative drainage, spleen heat, portal vein thrombosis and bleed- ing, hospitalization days, while the operation time and blood loss presented significant difference （P〈0.001）. Conclusion： LigaSure in pa- tients with portal hypertension splenectomy, pericardial paraesophsgastric devascularization application is safe and effective, it can signif- icantly shorten the operation time and reduce the amount of bleeding, and postoperative complications with the traditional separation liga- tion operation no difference.
