小瞳孔下白内障超声乳化摘除联合折叠式后房型人工晶体植入术的效果

摘　　要：目的探讨小瞳孔状态下的白内障超声乳化摘除联合折叠式后房型人工晶体植入术的手术技巧、注意事项和临床效果。方法对临床确诊的小瞳孔白内障患者23例(30眼),经巩膜隧道切口采用钝性分离等方法扩大瞳孔、环形撕囊,行双手原位超声乳化摘除联合折叠式后房人工晶体植入术。结果术后随访3～12个月,术后视力≥0.3者24眼占80.0%,所有患者术后瞳孔均恢复圆形或近圆形,主要并发症包括角膜基质层水肿、前房纤维素性渗出、后囊破裂等。结论合理选择瞳孔扩张技术,娴熟地手术技巧可使小瞳孔状态下的白内障患者于术后恢复生理性圆形瞳孔,无严重不良反应。Objective To explore the surgical techniques and the clinical effects of phacoemulsification and foldable posterior chamber intraocular lens （IOL） implantation in microcoria cataract. Methods 23 cases （30 eyes） of mierocoria cataract were per- formed with phacoemulsification and fodable posterior chamber IOL implantation by both hands through sclerocomeal incision after manu- al pupilloplasty and continuous circular capsulorhexis （CCC）. Results The visual acuity was improved significantly after operation. Fol- lowing 3 to 12 months, the visual acuity in 24 eyes were ≥0.3 （80.0%） and all the pupils became normal or almost - normal. Corneal e- dema,fibrous exudation in the anterior chamber and posterior capsule rupture were the major complications of the surgery. Conclusion The patients who have underwent phacoemulsification with muhifocal IOL implantation in microcoria cataract can restore physiological round pupils without severe side effects by using appropriate magnification strategy and skilled surgical technique.
