凉血散瘀汤加味治疗非增殖期糖尿病视网膜病变的临床观察

摘　　要：目的评价凉血散瘀汤加味治疗非增殖期糖尿病视网膜病变(Nonproliferative Diabetic Retinopa-thy,NPDR)的临床疗效。方法将确诊为瘀热阻络证型的糖尿病视网膜病变非增殖期患者56例随机分为中医治疗组和对照组。两组患者在均接受糖尿病药物及羟苯磺酸钙治疗的情况下,中药治疗加服凉血散瘀汤加味,连续服药3个月。两组治疗前后分别进行荧光素眼底血管造影(Angiography Fiurescence Fundus,FFA)、血液流变学指标、视野平均敏感度(Mean Sensitivity,MS)及视网膜电图震荡电位(Electroretinogram OscillatoryPotentials,ERG-OPs)检查,并进行总体疗效评价。结果治疗3个月后,中医治疗组患者血液流变学各项指标均低于对照组,差异有统计学意义(P<0.05);中医治疗组平均光敏感度好转程度优于对照组,差异具有统计学意义(P<0.05);中医治疗组ERG-OPs波峰潜伏期均缩短,振幅明显提高,治疗前后比较差异均具有统计学意义(P<0.05),中医治疗组患者ERG-OPs波峰潜伏期及振幅改善程度优于对照组,差异均有统计学意义(P<0.05)。中医治疗组总体疗效优于对照组(P<0.05)。结论凉血散瘀汤加味可以降低NPDR患者的血液粘度,提高视野平均敏感度,改善视网膜电图震荡电位指标。OBJECTIVE To evaluate the effects of modified Liangxue Sanyu decoction in nonproliferative diabetic retinopathy patients. METHODS Fifty-six cases with nonproliferative diabetic retinopathy （NPDR） who were diagnosed with the syndrome of stasis-heat obstructing in the collateral channels were randomly divided into the observation group and the control group. Both groups were treated with diabetic medication and calcium dobesilate capsules. Modified Liangxue Sanyu decoction was added in the observation group. Administered these drugs to the patients for 3 months continuously and the changes in FFA, hemarheology test, MS and ERG-OPs were observed be- fore and after treatment. RESULTS The hemorheology indexes of observation group after the treatment at three months were all lower than those of control group, and there were significant differences （P〈0.05）. After treatment, the mean sensitivity in two groups were improved, and there were statistical differences between two groups （P〈 0.05）. The latency of ERG-OPs in observation group was shortened, and the total amplitude was increased. There were statistical differences before and after treatment （P〈0.05）, and statistical differences between two groups （P〈 0.05 ）. Three months after treatment the overall efficacy of the observation group was superior to that of control group （P〈0.05）. CONCLUSIONS Modified Liangxue Sanyu decoction could significantly reduce blood viscosity, increase MD, improve ERG-OPs indexes, be good at high efficiency and be worth popularizing application in clinic.
