介入支架成形术对椎-基底动脉狭窄62例治疗作用观察分析

摘　　要：目的评估采用支架成形术(CAS)对椎-基底动脉狭窄的治疗作用。方法自2006年4月至2009年2月,62例患者(48～79岁,平均65岁),主诉后循环缺血(PCI)症状如眩晕发作,经数字减影血管造影(DSA)确诊为椎-基底动脉狭窄。接受经皮腔内CAS,采用球囊扩张支架对椎-基底动脉狭窄段行球囊扩张CAS,支架安放位置为致血流灌注不足的动脉狭窄处。结果 62例患者共放置69枚支架,手术成功率为100%。术后DSA显示狭窄段全部成形良好,椎-基底动脉狭窄段狭窄程度从术前的(71.5±9.2)%降至(5.6±2.2)%。无介入相关性死亡病例发生。随访6～38个月,平均(20.6±5.8)个月,患者临床症状消失51例,改善8例,复发3例。围术期无严重并发症发生。结论 CAS治疗椎-基底动脉狭窄是一种安全、有效的方法。Objective To assess the effect of carotid angioplasty and stenting （CAS） in treating vertebrobasilar artery stenosis. Methods A total of 62 patients （aged 48-79 years old, average 65 years old） with the chief complaint of posterior cir- culation ischemia（PCI） symptoms like vertigo onset and diagnosed with vertebrobasilar artery stenosis by digital subtraction an- giography （DSA） from April 2006 to February 2009 received the percutaneous transluminal CAS therapy. The balloon dilatation was conducted in vertebrobasilar artery stenosis segment by balloon expandable stent. Results 69 stents were implanted in 62 patients with the operation success rate of 100%. Postoperative DSA showed that all forms in narrow segment were good. The vas- cular stenosis degrees of vertebrobasilar artery stenosis were decreased from preoperative （71.5±9.2）% to postoperative （5.6±2.2）%. No intervention-related death case ocurred. Follow up lasted for 6-38 months, average （20.6±5.8）months. The clinical symptoms were disappeared in 51 cases, relieved in 8 cases and recurred in 3 cases. There was no severe complications during perioperative period. Conclusion CAS is safe and effective in the treatment of vertebrobasilar artery stenosis.
