注射用薏苡仁油联合吉西他滨治疗老年晚期胰腺癌的临床观察

摘　　要：目的评价注射用薏苡仁油联合吉西他滨治疗老年晚期胰腺癌的临床疗效和不良反应。方法 47例大于65岁的晚期胰腺癌患者随机分为治疗组(24例)和对照组(23例),2组均用吉西他滨化疗,治疗组同时加用注射用薏苡仁油,3周为1个周期。结果治疗组和对照组有效率分别为36%和23%,2组比较无显著性差异(P>0.05);临床受益反应率分别为71%和41%,2组比较有显著性差异(P<0.05)。其中治疗组对中重度癌痛控制率为73%,对照组为46%,2组比较有显著性差异(P<0.01);治疗组和对照组中位疾病进展时间(TTP)分别为6.3个月和4.2个月,2组比较有显著性差异(P<0.05);中位生存时间(OS)分别为7.5个月和6.7个月,无显著性差异(P>0.05)。2组不良反应以血液毒性、消化道反应为主,治疗组3～4级中性粒细胞减少、血小板减少均低于对照组(P均<0.05)。结论注射用薏苡仁油联合吉西他滨治疗老年晚期胰腺癌安全、有效,在改善生活质量,特别是控制癌痛方面具有一定优势。Objective It is to evaluate the effect and adverse reactions of coicis oil injection combined with gemcitabine in treating old patients with advanced pancreatic cancer. Methods Fourty-seven old patients with advanced pancreatic cancer were randomly divided into treatment group （24 cases） and control group （23 cases）. Both groups were treated by gemeit- abine, and the patients were also given coicis oil injection in treatment group, 3 weeks was a cycle in both two groups. Re.suits The overall response rate was 36% in treatment group and 23% in control group （ P 〉 0.05 ） , Clinical benefit rate was 71% in treatment group and 39 % in control group （P 〈 0.05）. The control rate of modest and severe degree pain was 73% in treatment group and 46% in control group （P 〈 0. Ol ）. The me＂dian time to progression was 6.3 months in treatment group and 4.2 months in control group （ P 〈 0.05 ） , and median overall survival were 7.5 months in treatment group and 6.7 months in control group （ P 〉 0.05 ）. The main toxicities were myelosuppression, gastrointestinal tract reaction. A lower rate of grade 3 and 4 myelosuppression were occurred in treatment group compared with that of control group （ P 〈 0.05 ）. Conclusion Coicis oil injection combined with gemcitabine regimen is secure and effective for old patients with advanced pancreatic cancer, and it also can elevate the life quality and alleviate pain.
