角化棘皮瘤15例病理分析

摘　　要：目的探讨角化棘皮瘤的病理学特征、恶性程度及预后。方法回顾性分析2010年—2012年我科15例角化棘皮瘤患者的资料,观察和探讨其组织病理学特征。结果角化棘皮瘤的典型病变为境界清楚的半球状隆起,中央溃疡形成呈现"火山口"样;组织病理以表皮向真皮内凹陷呈"火山口"状,其中充满角化物质,肿瘤两侧的上皮细胞形成鱼唇样特征;应选择手术治疗,长期随访。结论角化棘皮瘤为低度恶性的皮肤肿瘤,有特殊的临床及组织病理特点,特别要注意与皮肤鳞状细胞癌鉴别。Objective To study the clinical and pathological features,malignancy and prognosis of keratoacanthoma.Methods Collect fifteen patients with keratoacanthoma,analyze and observation of its clinical features and pathological characteristics.Results The typical lesion of keratoacanthoma is a hemispheric,domeshaped,skincolored nodule in which there is a crateriform ulcer with a central keratin plug.Histological examination showed the crateriform ulcer filled with Keratinized material.A＂lip＂edge formed by invagination of epiderm is surrounds the keratin—filed crater.Wide excision of the lesions is the first choice for the treatment.Conclusions keratoacanthoma is a skin malignant tumor with it＇s typical clinical and pathological features.Differential diagnosis should be addressed in the case of squamous cell carcinoma in skin.
