针刺对部分前循环脑梗死患者疾病严重程度及日常生活能力的影响

摘　　要：目的观察针刺治疗部分前循环脑梗死偏瘫患者的临床效果。方法将120例部分前循环脑梗死偏瘫患者随机分为对照组、针刺组、康复组、针刺+康复组各30例。对照组予丹红注射液及依达拉奉注射液静脉滴注(疗程2周)、口服拜阿司匹灵片(疗程6个月),并对症治疗。针刺组在对照组治疗基础上给予针刺治疗,康复组在对照组治疗基础上给予康复治疗,针刺+康复组在对照组治疗基础上给予针刺和康复治疗,疗程均为3个月。分别评定各组治疗前、治疗后第1、3、6个月美国国立卫生研究院卒中量表(NIHSS)及日常生活能力评定量表(BI)评分。结果除对照组治疗后第1个月NIHSS评分差异无统计学意义(P>0.05),其余各组治疗后不同时间NIHSS评分、BI评分均较本组治疗前有明显改善(P<0.05)。治疗后第1、3、6个月对照组与其他3组同时间点比较差异均有统计学意义(P<0.05)。针刺组与康复组在治疗后1个月NIHSS评分、BI评分差异有统计学意义(P<0.05),针刺组优于康复组;治疗后第3、6个月NIHSS评分、BI评分差异均无统计学意义(P>0.05)。针刺+康复组与针刺组、康复组在治疗后1、3、6个月NIHSS评分、BI评分差异均有统计学意义(P<0.05),针刺+康复组明显优于针刺组和康复组。结论针刺可促进部分前循环脑梗死患者疾病严重程度及日常生活能力的恢复,其联合康复手法则效果更好。Objective To observe the rehabilitation effect of acupuncture for hemiplegic patients with partial anterior circulation infarction （PACI）. Methods Totally 120 hemiplegic patients with PACI were randomized into the control group, acupuncture group, rehabilitation group and acupuncture ＋ rehabilitation group, with 30 in each. The control group was intravenously injected with Danhong Injection and edaravone injection for 2 weeks, orally taken bayaspirin tablets for 6 months and received symptomatic treatment. The acupuncture, rehabilitation and combo therapy of acupuncture and rehabilitation was added in the acupuncture group, rehabilitation group and acupuncture ＋ rehabilitation group respectively on the basis of control group. The acupuncture and rehabilitation lasted for 3 months. The National Institutes of Health Stroke Scale （NIHSS） and Barthel Index （BI） were used to evaluate the efficacy and activities of daily living before treatment and after 1-month, a-month and 6-month treatment. Results The scores of NIHSS and BI were significantly improved in three treatment groups at different time points after treatment （P〈0.05） expect the NIHSS score in the control group after 1-month treatment （P〈0. 05）. There were significant differences between the control group and other three groups after 1-month, a-month and 6-month treatment （P〈0.05）. The scores of NIHSS and BI in the acupuncture group were superior to those in the rehabilitation group after 1-month treatment （P〈 0. 05）. There was no significant difference in the scores of NIHSS and BI between the acupuncture group and rehabilitation group after a-month and 6-month treatment （P〈0.05）. The scores of NIHSS and BI in the acupuncture ＋ rehabilitation group were superior to those in the acupuncture group and rehabilitation group after 1-month, a-month and 6-month treatment （P〈0.05）.
