健康体检人群血清25-羟基维生素D水平与非酒精性脂肪肝的相关性

摘　　要：目的探讨健康体检人群血清25-羟基维生素D(25OHD)与非酒精性脂肪肝(NAFLD)的关系。方法采用电化学发光法测定204例非酒精性脂肪肝体检者(NAFLD组)及130名无脂肪肝的健康体检者(对照组)的血清25OHD水平,比较2组各测量指标的差异。按照维生素D缺乏标准将所有受试者分为25OHD<10ng/mL组和25OHD≥10ng/mL组,比较2组受试者的年龄、性别、NAFLD发病情况及胰岛素抵抗指数间的差异。用t检验或者卡方检验比较2组间各指标间的差异,用Pearson相关分析血清25OHD水平与各参数之间的相关性。结果 NAFLD组收缩压、体重指数(BMI)、腰臀比(WHR)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、尿酸、空腹血糖(FPG)和胰岛素抵抗指数(HOMA-IR)均高于正常对照组,血清25OHD水平低与正常对照组,差异有统计学意义(均P<0.05)。血清25OHD缺乏多见于老年人和NAFLD患者,25OHD缺乏人群BMI及HOMA-IR明显高于25OHD≥10ng/mL人群,差异有统计学意义(P<0.05)。相关分析结果显示,NAFLD组25OHD与BMI(r=-0.306,P<0.01)、HOMA-IR(r=-0.480,P<0.01)呈负相关,经偏回归校正后,25OHD与HOMA-IR仍然相关。结论血清低25OHD水平与NAFLD相关。Objective To investigate the relationship between serum 25OHD level and nonalcoholic fatty liver disease （ NAFLD） in physical examination population. Methods Serum 25OHD level was measured by electrochemical luminescence method in the 204 NAFLD groups and the 130 health control groups. The difference about the measurement index between the NAFLD and the health control groups were evaluated. The difference about the sex / age / incidence of NAFLD and the insulin resistance between the 25OHD 10 ng / mL with 25OHD≥ 10 ng / mL were also evaluated. The measurement data used t test. The enumeration data used χ^2 test. The correlation compare used Pearson analysis. Results The Measurement index,such as BMI,WHR,HOMA-IR,was obviously increased in patients with NAFLD,while patients with NAFLD had reduced serum 25OHD levels compared to subjects without NAFLD （ P 0. 05） . The dificiency of serum 25OHD was more often in the elder people （ age 60 years） and people with nonalcoholic fatty liver disease. BMI and HOMA-IR in the people with serum 25OHD 10 ng / mL was obviously higher than that in the people with serum 25OHD ≥10 ng / mL. There is a negative correlation between 25OHD with BMI （ r = 0. 306,P 0. 01 ） and HOMA-IR （ r = 0. 480,P 0. 01 ） . Conclusion Low 25OHD levels are associated with the presence of NAFLD.
