多发性骨髓瘤患者希望水平及影响因素的调查

摘　　要：目的了解多发性骨髓瘤患者的希望水平，并探讨其影响因素。方法通过便利抽样，对某三甲医院103例多发性骨髓瘤住院患者采用Herth希望量表和自制一般情况调查表进行调查。结果多发性骨髓瘤患者的希望总分为23—48分，平均为（35．56±5．34）分，49．5％的患者处于低中等水平，对现实和未来的积极态度、采取积极的行动、与他人保持亲密的关系维度得分分别为8—16，6～16，8～16分；对自身疾病了解程度不同、当前所处疗效阶段不同、医疗费用对家庭影响不同患者希望水平差异有统计学意义（t分别为4．678，5．590，11．80；P〈0．05）；不同婚姻状况、工作状况、睡眠质量患者及家属是否经常探望患者其希望水平差异均有统计学意义（t分别为3．012，5．583，9．701，3．135；P〈0．01）；him患者的希望水平影响因素的多元回归分析显示，医疗费用对家庭的影响和自身睡眠质量是多发性骨髓癌患者希望水平的显著影响因子，可以解释希望水平29．1％的变异量医疗费用对家庭的影响和自身睡眠质量是多发性骨髓癌患者希望水平的显著影响因子，可以解释希望水平29．1％的变异量。结论多发性骨髓瘤患者的希望水平处于中等水平。睡眠质量与医疗费用对家庭的影响是患者希望水平的预测因子，护士应充分评估这些因素，以便采取针对性心理护理措施，提高其希望水平。Objective To investigate the hope level in patients with multiple myeloma and discuss its influencing factors. Methods 103 cases of patients with multiple myeloma were selected by convenience sampling and investigated with Herth Hope Scale and self-designed questionnaire. Results The total score of hope in patients with multiple myeloma was 23-48, and the average score was （35.56±5.34）. 49.5% patients were of the low or moderate level, and their scores in dimensions including positive attitude toward reality and future, positive effect, and intimate relationship with others were respectively 8 - 16, 6 - 16 and 8 - 16. Patients with different understanding of their illness, different stages of curative effect and different influence of medical cost on families had significantly different hope level （ t = 4. 678, 5. 590, 11.80, respectively ; P 〈 0.05 ）. Patients of different marriage status, working status, sleep quality and family visit also had significantly different hope level （t = 3. 012, 5. 583, 9. 701, 3. 135, respectively; P 〈 0.01 ）. Multiple linear regression analysis showed that influence of medical cost on families and sleep quality were the main influencing factor of the hope level in patients with multiple myeloma, which could explain 29. 1% of the hope level variation. Conclusions The hope level in patients with multiple myeloma is in the moderate level. Sleep quality and influence of medical cost on families are the influencing factors of their hope level, which nurses should assess carefully enough to take psychological nursing measures to improve their hope level.
